Abstract [sv]
Rupture of abdominal aortic aneurysm is a common cause of death in Europe and North-America. Since most patients who die from rAAA lack symptoms prior to rupture the only available method to decrease mortality is through screening. Large RCTs showed that AAA-associated mortality was reduced by 60-70% by screening with ultrasound, but important issues regarding design remain controversial. The first AAA screening program in Sweden was started in April 2006 in Uppsala, offering a unique possibility for investigation. Identifying small AAAs not only offers the opportunity to operate on patients before rupture, but also to inhibit expansion, if etiology and pathogenesis is known. However, more than 90% of the patients with AAA have unknown etiology. The project focuses on studying inflammation (with PET/CT with novel tracers), specific infections, coagulation, fibrinolysis and cardio-vascular risk-factors in patients with small and large AAAs, as well as with multiple aneurysm disease. A crucial ethical aspect is that elective AAA surgery is associated with mortality. The two major causes of death after AAA-surgery are myocardial infarction and multiple organ failure, associated with intestinal ischaemia. The project focuses on studying pre- and perioperative myocardial injury (with MRI) as well as specific laboratory markers for intestinal ischaemia. The research group consists of 15 senior investigators, 12 registered and 4 future PhD-students, as well as many collaborators.
