Implementation of Self-medication in a Geriatric Unit – Healthcare Providers’ and Patients’ Perspectives (engelsk)
Abstract [en]

Background and objective: Non-compliance can result from patients not being involved in their medication during their hospital stay. Self-administration of medications can improve patients’ medication knowledge, compliance and autonomy. The aim of this study was to explore whether implementation of self-medication is possible in geriatric medical unit 30A, by assessing the perspective of healthcare providers and patients in this unit as well as collecting previous experiences from a hospital where self-medication is already in use today.

Study design: In this qualitative descriptive study, a survey was given to suitable patients to discover their opinions on self-medication. In two semi-structured focus group interviews, nurses and physicians discussed their perceptions on the subject. A qualitative content analysis was performed on the data. Previous experiences of self-medication were obtained from a visit to a hospital in Jönköping.

 

Setting: This study was mainly conducted at unit 30A at Akademiska Sjukhuset in Uppsala, with a visit to Länssjukhuset Ryhov in Jönköping.

 

Main outcome measures: Reasons for patients wanting or not wanting to try self-medication were collected. Common themes discussed in the interviews were aspects related to healthcare providers, patient safety and autonomy.

 

Results: Of 30 patients that filled in the survey, 13 wanted to try self-medication in order to improve medication knowledge and autonomy. Letting go of responsibility was a main reason for patients not wanting to try. Physicians and nurses were generally positive to self-medication. Potential benefits mentioned were timesaving for nurses and increased autonomy for patients. Potential disadvantages included difficulties in determining suitable patients, impaired patient safety and nurses’ control issues. According to previous experiences, communication between staff and patients and suitable patients managing suitable medications is important for successful self-medication.

 

Conclusions: For successful implementation in 30A, a routine for self-medication is crucial, and also that patients are able to manage their medications and that nurses can let go of their medication responsibility. Self-medication can potentially lead to improved medication knowledge, compliance and autonomy for patients in 30A, but it can be further discussed whether implementation is worth the effort for only a small number of patients.
