Abstract [en]

Transient left ventricular dysfunction in patients under emotional or physiological stress, also known as takotsubo-cardiomyopathy, has recently been recognised as a distinct clinical entity. The syndrome is characterised by an onset resembling acute myocardial infarction and reversible wall motion abnormalities involving the left ventricular apex and mid-ventricle, but without angiographic evidence of coronary artery stenosis. In this report we present two cases and review the clinical characteristics and potential pathophysiological mechanisms of the takotsubo-cardiomyopathy.

Takotsubo-kardiomyopati beskrevs första gången 1990 i Japan.

Tillståndet karakteriseras av ett insjuknande som liknar akut hjärtinfarkt i kombination med en uttalad men reversibel nedsättning av den systoliska funktionen i vänster kammares apikala och mellersta segment, dock utan att man finner några signifikanta kranskärlsförändringar.

Postmenopausala kvinnor förefaller löpa störst risk att drabbas.

Emotionell eller fysiologisk stress föregår ofta insjuknandet.

Prognosen tycks vara god även om enstaka dödsfall har beskrivits.
