Botoxinjektioner i stämbanden, EMG- respektive fiberskopiledda: en jämförande studie

Abstract [sv]

Adductor spasmodisk dysfoni och rösttremor är två röststörningar som vanligen behandlas med injektioner av botulinumtoxin i stämbanden. Injektionerna kan ges på två sätt, med ledning av fiberskopi eller EMG. Syftet med studien var främst att undersöka om de båda metoderna gav en skillnad i behandlingseffekt gällande röstkvalitet, rösthandikappindex, behandlingseffektens längd samt patienternas upplevelse av obehag. Även behandlingseffekt av botulinumtoxininjektioner oberoende av metod undersöktes. Studien utvärderade insamlad data från 27 patienter med diagnosen adductor spasmodisk dysfoni och/eller rösttremor. Resultaten visade inte på några skillnader i behandlingseffekt mellan de båda metoderna. Patienterna som fick EMG-ledda injektioner skattade obehaget något högre än patienterna som fick fiberskopiledda injektioner, men validiteten hos denna skillnad kan ifrågasättas.  Studien tyder på (a) att botulinumtoxininjektioner har en god effekt vid behandling av adductor spasmodisk dysfoni och rösttremor, (b) att det inte finns någon skillnad i denna effekt mellan de båda metoderna samt (c) att injektionerna, oberoende av metod, ger relativt lite obehag för patienterna.

Abstract [en]

Adductor spasmodic dysphonia and voice tremor are two voice disorders that commonly are treated with botulinum toxin injections in the vocal cords. The injections can be performed in two ways, guided either by fiberscopy or EMG. The purpose of the study was primarily to investigate if there was a difference in effect of treatment between the two methods concerning quality of voice, voice handicap index, length of effect of the treatment and the patients’ experiences of discomfort. The effect of treatment by botulinum toxin injections, independent of used method, was also analyzed. The study evaluated collected data from 27 patients diagnosed with adductor spasmodic dysphonia and/or voice tremor. The results did not indicate any differences in effect of treatment between the two methods. The patients that were given injections guided by EMG rated the discomfort slightly higher than the patients that received injections guided by fiberscopy, but the validity of this difference can be questioned. The study indicates (a) that botulinum toxin injections has a good effect when treating patients with adductor spasmodic dysphonia and voice tremor, (b) that there are no differences in this effect between the two supportive methods and (c) that  the injections, independent of used method, give the patients relatively low degree of discomfort.
