Abstract [en]

The aim with this study was to investigate and describe the nurses administration of medication on a surgical ward. Method: the study is formed as an observation- and patient record auditing study. The auditing was done by two nurse students during two days on a surgical ward at a university hospital. The students audited the nurses in their medication and administration of drugs to patients, the administration of 100 drug doses was observed. The handling and administration was observed and registered with a protocol, formed by Collaborative Alliance of Nursing Outcomes (CALNOC), for increased quality of medication. Result: The majority (89 %) of the distributed doses were distributed without any errors when comparing the observation notes to the journal. The most common errors observed during the observation was lack of verifying the patient’s identity, lack of explanation and labeling the drugs. Most nurses documented before the drug was administrated. Conclusion: The result means non-compliance, according to both CALNOC and the Swedish Social Board's General Council. Following this study more studies of intervention are considered to be implemented.

Syftet med denna studie var att undersöka och beskriva läkemedelshanteringen utifrån sjuksköterskans roll på en kirurgisk vårdavdelning. Metod: Arbetet utfördes som en observations- och journalgranskningsstudie. Under tre dagars tid var två sjuksköterskestudenter på en kirurgisk vårdavdelning vid ett universitetssjukhus för att observera sammanlagt 100 läkemedelsdoser. Sjuksköterskornas läkemedelshantering observerades och jämfördes sedan mot ordinationshandling. Collaborative Alliance of Nursing Outcomes (CALNOC) kvalitetsindikatorer (process och resultat) för läkemedelshantering användes. Resultat: Majoriteten (89 %) av de doser som delades ut, gjordes utan att något fel påträffades, dvs. patienten fick de läkemedelsdoser som var ordinerade. De vanligaste bristerna som förekom sågs under observationerna. Utebliven identitetskontroll, bristande förklaring till patienten och märkning av läkemedlet samt att de flesta sjuksköterskorna dokumenterade innan läkemedlet administrerades tillhör de mest observerade bristerna. Slutsats: Resultatet visar på bristande följsamhet, enligt både CALNOC och Socialstyrelsens allmänna råd. Efter denna studie anses att fler studier med interventioner genomförs samt att Socialstyrelsen allmänna råd och riktlinjer förtydligas så att de blir lättare att följa.
