Abstract [en]

Aim: To examine the ethical problems intensive care unit nurses face when working with seriously ill patients, whether they think they have the necessary skills to handle such situations and if there is a need for ethical discussions in the working group.

Method: Study of qualitative descriptive character. Interviews were conducted with eight intensive care nurses in an intensive care unit at a major hospital in central Sweden. Interviews were transcribed and analyzed using manifest content analysis.

Findings: The intensive care nurses reported that ethical dilemmas arise related to decisions whether to terminate treatment, determining how much time and resources to invest in patients, determining which patients should be prioritized, in situations when patients are moved to the ward or between intensive care units before treatment is completed. Nurses reported that they felt competent to handle ethical difficulties, based on experience. While there were no guidelines on how to approach ethical dilemmas, most thought it would be difficult to establish general guidelines as each situation is unique. All participating nurses agreed that there is a need for ethical discussions, that all professionals should participate in such discussions, and those discussions should occur on a continuous basis.

Conclusions: Intensive care nurses are faced with ethical dilemmas and the nurses felt that there is a need to conduct ethical discussions among all professions within the team. These discussions should, according the nurses, be scheduled continuously and if necessary when an acute ethical dilemma arises.
