Abstract [sv]

Syfte: Att beskriva samt jämföra bemanningstal inom den svenska dialysvården samt beskriva relation mellan bemanning och kvalitetsutfall för behandling av anemi. Metod: Samtliga 78 dialysenheter i Sverige erhöll förfrågan om att medverka i studien, 40 dialysenheter deltog. Datainsamling med strukturerad telefonintervju. Deskriptiv statistik bearbetades. Med Spearmans rangkorrelation undersöktes samband mellan bemanning och måluppfyllelse av hemoglobinvärde och järnmättnad. Mann Whitney U test användes för att jämföra olika dialysenheter med varandra om det fanns skillnader i hemoglobinvärde och järnmättnad beroende på om de hade anemiansvarig sjuksköterska, läkare, sjukgymnast och dietist. Resultat: Studien gav en bild över hur bemanningen såg ut på 40 av Sveriges dialysenheter, samt hur många patienter som vårdas under ett förmiddagspass. Det kunde inte påvisas något signifikant samband mellan bemanning och måluppfyllelse för hemoglobinvärde och järnmättnad. Inte heller några signifikanta skillnader mellan de dialysenheter som hade anemiansvarig sjuksköterska eller läkare, sjukgymnast och dietist tillgänglig varje dag och de som inte hade det gällande måluppfyllelse av hemoglobinvärde och järnmättnad. Slutligen fanns ingen skillnad mellan större och mindre dialysenheter. Slutsats: Bemanningen inom dialysvården i Sverige varierade. Inga signifikanta samband eller skillnader kunde redovisas i studien men vidare studier uppmuntras.

Abstract [en]

Aim: To describe and compare  staffing numbers in dialysis units in Sweden and to describe the relationship between staffing and quality outcomes regarding treatment of anemia. Method: A national sampling procedure was used and 40 dialysis units participated. Data collection was conducted using structured telephone interviews. Data was analysed using SPSS. Descriptive statistics were processed. The Spearman rank correlation was used to investigate the correlation between staffing and achievement of hemoglobin level and iron saturation. Mann Whitney U test was used to compare different dialysis units with each other and if there were differences in hemoglobin level and iron saturation depending on whether they had nurses in charge of anemia management, or a doctor, physiotherapist and dietician avalilable to the unit every day. Results: The study could provide a picture of staffing at 40 dialysis units in Sweden, and the number of patients cared for during a morning session. The study could not demonstrate any significant relationship between the staffing and achievement of hemoglobin level and iron saturation. Furthermore, no significant differences between the dialysis units who had nurses in charge of anemia management, or doctor, physiotherapist and dietician available every day and those who did not, in the fulfillment of hemoglobin level and iron saturation. There was no difference between larger and smaller dialysis units either. Conclusion: Staffing in Swedish dialys units varied. No significant correlations or differences were reported in the study, but further studies are encouraged.
