Upplevd hälsa hos personer med typ-2 diabetes som äter Low Carb High Fat (LCHF)-kost

Bakgrund: Det har varit mycket diskussioner om vilken kost som är lämplig vid typ-2 diabetes. LCHF-kost har på senare tid blivit uppmärksammad som ett bra alternativ. Det finns flera vetenskapliga studier avseende de fysiologiska effekterna av LCHF-kost, men inte på den självupplevda hälsan hos dem som följer kosten. Syfte: Att undersöka upplevd hälsa hos personer med typ-2 diabetes som äter enligt LCHF-kost och vilket bemötande de fått av sjukvårdspersonal relaterat till kostvalet. Ett ytterligare syfte är att undersöka var dessa personer hittat kunskap om LCHF-kost. Metod: Kvalitativ intervjustudie. Telefonintervjuer hölls med tio medlemmar ur Facebook- gruppen ”Olydiga DIABETIKER” som anmälde sig frivilligt till studien. Resultat: Deltagarna har märkt flera hälsofördelar sedan de börjat med LCHF-kost; förbättrat HbA1C, viktnedgång, förbättring av övriga sjukdomstillstånd, exempelvis smärtande leder och hypertoni, och de mår generellt mycket bättre. Upplevelsen av att inte bli respekterad för sitt kostval beskriver flertalet intervjupersoner och de önskar att sjukvårdspersonal ska ta till sig mer av den nya kunskapen om LCHF-kost. Hur de funnit kunskap om LCHF-kosten varierade mellan att ha läst själv, fått tips och en kombination av de två. Slutsats: Deltagarna i studien upplever sin hälsa som avsevärt förbättrad efter start av LCHF- kost. De är missnöjda med bemötandet de fått från sjukvårdspersonal angående sitt kostval, vilket tyder på att sjukvårdspersonal behöver bli bättre på att tillämpa personcentrerad vård. Det finns ett behov av vidare forskning inom detta område. 

Abstract [en]

Background: There is an ongoing debate concerning the dietary treatment of type 2 diabetes. The LCHF diet has recently been recognized as a viable alternative to more traditional diets. Research has been made on the physiological effects of the LCHF diet, but not on the subjective feeling of health. Objective: To study the subjective feeling of health in persons with type 2 diabetes that follow the LCHF diet and how they are treated by medical personnel in relation to their choice of diet. Another aim was to find out where they gained their knowledge of the diet. Method: Qualitative interview study. Interviews were held over the phone with ten members of the Facebook community “Olydiga DIABETIKER” who volunteered to participate in the study. Results: The participants experienced several health benefits since they started following the LCHF diet, such as improved HbA1C, weight loss, improvement of other medical conditions such as aching joints, hypertension and improved general state of health. Several participants felt that their choice of diet was not respected by medical personnel and that they wanted them to acknowledge new evidence of the effects of the LCHF diet. The participants gained their knowledge of the diet in different ways. Conclusion: The participants perceive their health as considerably improved since they started following the LCHF diet. They are dissatisfied with how they are being treated by medical personnel concerning their choice of diet, which indicates that medical personnel need to get better at practicing person-centered care. Further research is required.
