I denna studie ställs frågan om den psykiatriska diagnosens sociala praktik för människor som inte har den psykiatriskt diagnostiserande specialvetenskapens utbildning genom att utforska ett perspektiv som inte uppmärksammats av forskningsfältet: vårdpersonalsperspektivet. Frågeställningen är hur människor som lekmän använder psykiatriska diagnoser och vilka faktorer som påverkar hur psykiatriska diagnoser upplevs som meningsfulla. Uppsatsens teoretiska utgångspunkt är hämtad från Ian Hackings analys av psykiatrisk diagnostik som en interaktiv kategori tillsammans med en analys av Martin Heideggers syn på det mänskliga ”varat” som ett socio-ontologiskt koncept. Härigenom framträder en bild av hur psykiatriska diagnoser är problemformuleringar sprungna ur självreflektion. Metoden som används är en explorativ intervjustudie med 26 vårdanställda i den svenska vården vilka i relation till den psykiatriska specialvetenskapen representerar ett aktivt lekmannaperspektiv på den psykiatriska diagnostiken och därmed representerar allmänhetens uppfattning om sjuklig normavvikelse. Resultaten visar att den psykiatriska diagnostiseringen är en kontextbunden social praktik där diagnoserna är meningsfulla i den utsträckning de har en praktisk funktion genom vilken de kan användas för att underlätta yrkesvardagen och hjälpa patienter att hantera det som av vårdpersonalen uppfattas som patientens problem. Det betyder att de faktorer som styr personalens och patienternas kontexter också styr diagnosernas sociala praktik samt att den part som dominerar kontexten alltid definierar vad som är det med diagnosen förknippade problem, dess symptom.

Abstract [en]

In this study the question of the social practice of psychiatric diagnosis is explored through the eyes of the health care worker, a perspective that has been overlooked in previous research. The question asked is how people who are laymen in relation to the psychiatric diagnosing specialists use psychiatric diagnoses and what factors and how these factors influence the use and meanings of diagnoses and make them meaningful outside the medically specialized psychiatric context. The theoretical starting point is taken from Ian Hackings analysis of psychiatric diagnostics as an interactive category together with an analysis of Martin Heidegger's view of the human "being" as a socio-ontological concept through which a picture emerges of how psychiatric diagnoses are problems formulations as a result of self-reflection. The method is 26 exploratory interviews with health care workers in the Swedish health care sector which in relation to the psychiatric medical science represents an active layman perspective on the psychiatric diagnosis, and thus a public perception. The results show that psychiatric diagnosis is a context-bound social practice in which the health care workers see diagnoses as meaningful when they have a practical function by which they can be used to facilitate their work or/and to help patients deal with what the health care workers perceive as the patients problems and thus be helpful in everyday life. The results also show that the dominant party in the social context where diagnoses are used is the one that decides what characteristics is regarded as symptoms associated with the diagnoses.
