Bakgrund: Anestesiteam larmas till akutmottagningen för att hjälpa till att vårda kritiskt sjuka patienter. Anestesiteamet arbetar vanligen på operationsavdelningen och är vana att arbeta i team. Teamwork beskrivs handla om ledarskap, kommunikation och samarbete, vilket inte är så lätt att uppnå i den tumulta miljön på akutmottagningen. Bristande kommunikation skulle kunna riskera patientsäkerheten och kan leda till ett vårdlidnade för patienten. Syfte: Att beskriva anestesiteamets teamwork på akutmottagningen med fokus på kommunikation. Metod: En icke deltagande observationsstudie bestående av 12 observationer av anestesiteamet genomfördes på en akutmottagning. Två analysmetoder har använts, en kvalitativ och en kvantitativ. Etnografisk metod användes för att analysera data och beskriva vad anestesiteamet gjorde, hur de kommunicerade samt hur de sammarbetade i förhållande till miljön på akutmottagningen. Den kvantitativa analysmetoden bestod av det validerade bedömningsprotokollet TEAM som användes för att analysera anestesiteamets prestation. Resultat: Det framkom tre teman som beskrev anestesiteamets teamwork och kommunikation. Växla mellan ta initiativ och vara beredd, prata eller inte prata och vårda i en ovan miljö. Observationer där väl fungerande teamwork finns beskrivet kunde ett samband ses med höga poäng i bedömningen enligt bedömningsprotokollet TEAM. Slutsats: Anestesiteamets teamwork visade sig genom ett sammarbete där teammedlemmarna växlade mellan att ta initiativ och att vara beredda. Kommunikationen mellan teammedlemmarna kunde yttra sig i olika former och anestesiteamet anpassade sig väl till miljön på akutmottagningen.  Genom att kontinuerligt anpassa sig till omgivningen och de andra medlemmarna i anestesiteamet skulle detta kunna bidra till en ökad patientsäkerhet och minskad risk för uppkomst av vårdlidande. 

Abstract [en]

Background: Anesthesia teams’ are called to the emergency department to help care for critically ill patients. Anesthesia team members usually work at the surgical ward and are used to work in teams. Teamwork includes leadership, communication and cooperation, which is not easy to achieve in the tumultuous environment in the emergency department. Lack of communication may risk affecting patient safety negatively and in addition leading to suffering from care for the patient. Aim: To describe the anesthesia teams’ teamwork in the emergency department with focus on communication. Method: A non participant observational study including 12 observations of the anesthesia team was conducted in an emergency department. Two methods were used to analyze data, one qualitative and one quantitative. Ethnographic method was used to describe what the anesthesia team members were doing, how they communicated and how they cooperated in relation to the environment in the emergency department. The validated assessment protocol TEAM was used to assess the anesthesia teams’ performance. Result: Three themes were revealed that described the anaesthesia teams’ teamwork and communication. Shift between taking initiative and being prepared, speak or not to speak and care in an unfamiliar environment. In observations where well functioning teamwork was described a connection could be identified in the high scores according to the assessment protocol TEAM. Conclusion: The teamwork of the anesthesia team was shown through cooperation and how they changed between taking initiative and being prepared. Communication between the team members could be expressed in different forms and the anesthesia team adapted well to the environment at the emergency department. By constant adaptation to the environment and the other team members in the anesthesia team, this could contribute to increase patient safety and decrease risk for suffering from care.
