A survey of the drug use and adherence to national and local therapy recommendations among asthma and COPD patients at Falu health centre. (English)
Abstract [en]

Background and objective: Patients with the chronic diseases asthma and COPD have lifelong drug therapy to relive symptoms and minimize inconvenience due to the disease. It is therefore necessary to change the pharmacotherapy during the progression of the disease. In Landstinget Dalarna, a local recommendation of drugs is yearly published. This, among others, with an economical perspective. The aim of this survey was to look at the drug use and the adherence to national and local therapy recommendations at Falu health centre. The secondary aim was to see if it is any difference between asthma and COPD patients.

 

Study design: The study was conducted by reviewing medical charts from asthma patients that had been in contact with the health centre during 160101-160331 and from COPD patients that had been in contact during 150101-151231. Statistical differences between the groups were calculated with Chi-square test.

 

Setting: This observational study took place on Falu health centre in Falun, Sweden.

 

Main outcome measures: It was noted if the recommendations were adhered to “completely”, “partly” or “not”. Also the adherences to each drug group were noted.

 

Results: The asthma patients followed the national recommendations to 100%. The same number for the COPD patients was 71%. The COPD patients followed the national recommendations “partly” to 21% and “not” to 8%. The local recommendations were followed “completely” to 44%, “partly” to 22% and “not” to 34% in the asthma group. The same numbers for the COPD group were, 49%, 37% and 14% respectively. There was also a difference between the two groups both when in came to adherence to the local and national recommendations.

 

Conclusion: There was a difference when it came to following national recommendations between asthma and COPD patients. The difference might suggest a lack of knowledge about COPD treatment. There was also a difference when it came to following local recommendations. These deviations are however sometimes indicated.
