    Abstract [sv]

    Konkurrens är en viktig och vitt spridd styrform i dagens samhälle. Tillhandahållandet av allt från dagligvaror och bilar till sjukvård, skola och tv-program styrs i allt högre grad genom att utförare får konkurrera med varandra. Konkurrens har blivit en så vanlig styrform att den ofta ses som ett ”naturligt tillstånd”, som om det inte finns några alternativ.

    Huvudbudskapet i denna bok är att konkurrens inte är ett naturligt tillstånd. Konkurrens behöver oftast beslutas och organiseras fram, vilket innebär att saker och ting ofta behöver ändras för att det ska kunna bli konkurrens. Författarna beskriver hur konkurrens faktiskt fungerar, varför konkurrens införs, hur konkurrens skiljer sig från andra mer välkända styrformer och vilka resultat man kan vänta sig vid införandet av konkurrens som styrform. Till exempel förklaras varför konkurrens ofta leder till mer av samma i stället för ökad variation, och hur professioner som läkare och lärare påverkas av att konkurrera.

    Exemplen i boken kommer framför allt från offentlig sektor, men principerna fungerar lika väl för andra sektorer. Författarna menar att detta är värdefull kunskap för alla som är chefer, medarbetare eller kunder i organisationer som har konkurrensutsatts – vilket inbegriper de flesta i vårt samhälle.

    Abstract [en]

    This article deals with how organizations cope with institutional complexity. By applying theories of translation, we follow the lead of those arguing that attention has to be given to the microprocesses of institutional complexity. Thus far, empirical studies focusing on such processes have been rare. Among those studies undertaken, analyses of structures and practices have dominated while analyses of the interpretations and construction of meaning of involved actors have been neglected. We thus undertook an empirical study of the introduction of a national report on quality comparisons in health care. This report contained four conflicting institutional logics—a democratic logic, a professional logic, a managerial logic, and a market logic. We investigated the conflicts between these logics and how the conflicts were managed. The findings of the study contribute to institutional theory by suggesting that a new type of soft actor—the hybrid professional—is likely to be influential in organizations characterized by institutional complexity. The reason for this is found in the character and work of the hybrid professionals, where an important part of their work was to construct problems and solutions that aligned with all the logics at play.

    Rankningar och öppna kvalitetsredovisningar har blivit ett allt vanligare styrmedel inom såväl offentlig som privat sektor. I fokus för artikeln ligger Öppna jämförelser - ett nationellt system för öppna kvalitetsjämförelser i hälso- och sjukvården. Vi har studerat hur Öppna jämförelser har använts i styrningen i tre landsting och en region i Sverige och vad det har för konsekvenser för den professionella byråkratin som organisationsform samt för villkoren för professionellt yrkesutövande.

    Contemporary professions are increasingly challenged to open up to scrutiny from the outside. Earlier research is focused on two main types of responses and consequences: colonization by a managerial logic of self-monitoring and decoupling of auditing and professional practice. This paper describes a different type of response which implies that professionals get actively involved in monitoring their own activities, without losing their professional autonomy. Two cases from Swedish healthcare were investigated: accreditation at a hospital laboratory and the national quality registries. In both cases, professional involvement took the form of translation and negotiation in expert networks, restrained by a certain resistance towards external monitoring, but driven by an interest in legitimizing and developing professional work. The resulting situation is characterized as a 'soft autonomy' which combines professional internalization of originally non-professional auditing ideas with maintained professional control over evaluation criteria.

    På senare år har det tagits allt fler initiativ till att genomlysa sjukvården. Som exempel kan nämnas ackreditering, evidensbaserad medicin, WHO:s rankningar av olika länders sjukvårdssystem och mediernas jämförelser av olika sjukhus. Stora förhoppningar knyts till nya tekniska möjligheter att skapa öppenhet och utkräva ansvar.

    Den här boken undersöker strävan efter transparens i svensk sjukvård. Hur väl går det egentligen att återge sjukvårdskvalitet med några få enkla mått? vad synliggörs och vad kommer i skymundan? Transparens i sjukvården är varken oproblematisk eller gratis, men kraven på insyn kommer inte att minska, snarare tvärt om.

    Abstract [en]

    Organisational analyses of health care often characterise its constituent professions as building blocks, each having a clear-cut identity and well-defined task. The fact that professional structures rather than the work itself have been the focus of the literature on professional groups has reinforced this image. This thesis replaces this approach with one that takes into consideration the heterogeneity of occupational groups. Both the work performed and the structures that condition this work are explored. The content and diversity of the work physiotherapists perform and how this has changed over time has been investigated, as well as how the group co-operates with other occupational groups in given contexts.

    The results indicate that well-articulated professional boundaries are often a precondition for flexible co-operation, and that it is difficult to plan in advance for co-operation that involves crossing professional boundaries. In addition, the struggle of the professional group to control its work does not necessarily run counter to the demands of the organisation. Co-operation and the work itself create possibilities for controlling work.
