    Abstract [sv]

    Konkurrens är en viktig och vitt spridd styrform i dagens samhälle. Tillhandahållandet av allt från dagligvaror och bilar till sjukvård, skola och tv-program styrs i allt högre grad genom att utförare får konkurrera med varandra. Konkurrens har blivit en så vanlig styrform att den ofta ses som ett ”naturligt tillstånd”, som om det inte finns några alternativ.

    Huvudbudskapet i denna bok är att konkurrens inte är ett naturligt tillstånd. Konkurrens behöver oftast beslutas och organiseras fram, vilket innebär att saker och ting ofta behöver ändras för att det ska kunna bli konkurrens. Författarna beskriver hur konkurrens faktiskt fungerar, varför konkurrens införs, hur konkurrens skiljer sig från andra mer välkända styrformer och vilka resultat man kan vänta sig vid införandet av konkurrens som styrform. Till exempel förklaras varför konkurrens ofta leder till mer av samma i stället för ökad variation, och hur professioner som läkare och lärare påverkas av att konkurrera.

    Exemplen i boken kommer framför allt från offentlig sektor, men principerna fungerar lika väl för andra sektorer. Författarna menar att detta är värdefull kunskap för alla som är chefer, medarbetare eller kunder i organisationer som har konkurrensutsatts – vilket inbegriper de flesta i vårt samhälle.

    Abstract [en]

    The past decades have witnessed a radical transformation of the public sector in most of the Western world. Traditional methods of organising have gradually given way to new ideas of governance, inspired by and modelled on the private sector. This thesis focuses on the management accounting changes that have taken place in the Swedish health care sector. It addresses the questions of how different occupational groups have reacted to these changes and what impact they have had on the Swedish nurses' desire to strengthen their position as a profession. Three empirical studies were conducted to answer these questions. The first study focused on an incentive system at a thoracic clinic; the second on nurses in profit centres at a hospital; and the third was based on an analysis of the debate in the Swedish nurses' trade union journal concerning the introduction of purchaser-provider models in the Swedish health care sector. Two different theoretical perspectives on accounting were used to analyse the changes. The first perspective is institutional, viewing accounting as part of rationalised reform. In the second theoretical perspective employed, accounting is regarded as an important aspect of social and institutional practice.

    The studies showed that the impact of management accounting models on health care practice varied. The incentive system appeared to have no effect on the practices of the occupational groups at the thoracic clinic, whereas the profit-centre system at the hospital had several consequences for the nurses working there. The studies also showed that the new management accounting models both helped and hindered nurses' aspirations to strengthen their position as a profession. While nursing care suffered, formal responsibility for profit helped to reinforce the head nurses' status on the wards and quality assurance programmes made the nurses' work more visible.

    Abstract [en]

    This paper provides an account of the rising clamour for transparency in the Swedish healthcare field. Previous research into accounting, auditing and classification has shown that such visualizing technologies potentially have the capacity to create new objects and ideals in a field (Miller 1994; Power 1997; Bowker and Star 1999). It is thus reasonable to believe that the simultaneous use of various visualizing technologies in a field might transform it. This paper provides an analysis of the early stages of such a transformation process. The carriers that promote transparency are identified and it is shown that the drive for transparency has opened up the field to new actors. It is also shown that the drive for transparency has changed the quality discourse and as a consequence of that, the National Quality Registries are beginning to be opened up to the public.

    Abstract [en]

    This article analyses the private healthcare company Capio and its participation in the drive for transparency in the European healthcare field. An important point of departure for the paper is that technologies for transparency, such as accounting and auditing, are not neutral devices for increased openness, but carry with them programmatic dimensions that affect our norms and rules of how healthcare is to be organized and controlled. The drive for transparency engages different actors with various motives. To investigate this we carried out semi-structured interviews with 11 persons, mainly management members of Capio. We show that transparency in healthcare has been put forward by a private actor for strategic reasons. We argue that Capio's involvement in the drive for transparency should be seen as a ‘second-order strategy’ with the aim to create advantageous opportunities in a future European healthcare market. We show that Capio, through its propagation of various transparency technologies, has put forward programmatic ideals of industrialisation, marketisation and Europeanisation in healthcare. The main conclusion is that although Capio has engaged in the drive for transparency for business reasons, the company has also furthered certain political ideals in the field. This study contributes to the literature which problematizes the division between private and public, and between business and politics in healthcare, and is of interest to a broad health policy audience.

    This article deals with how organizations cope with institutional complexity. By applying theories of translation, we follow the lead of those arguing that attention has to be given to the microprocesses of institutional complexity. Thus far, empirical studies focusing on such processes have been rare. Among those studies undertaken, analyses of structures and practices have dominated while analyses of the interpretations and construction of meaning of involved actors have been neglected. We thus undertook an empirical study of the introduction of a national report on quality comparisons in health care. This report contained four conflicting institutional logics—a democratic logic, a professional logic, a managerial logic, and a market logic. We investigated the conflicts between these logics and how the conflicts were managed. The findings of the study contribute to institutional theory by suggesting that a new type of soft actor—the hybrid professional—is likely to be influential in organizations characterized by institutional complexity. The reason for this is found in the character and work of the hybrid professionals, where an important part of their work was to construct problems and solutions that aligned with all the logics at play.

    Rankningar och öppna kvalitetsredovisningar har blivit ett allt vanligare styrmedel inom såväl offentlig som privat sektor. I fokus för artikeln ligger Öppna jämförelser - ett nationellt system för öppna kvalitetsjämförelser i hälso- och sjukvården. Vi har studerat hur Öppna jämförelser har använts i styrningen i tre landsting och en region i Sverige och vad det har för konsekvenser för den professionella byråkratin som organisationsform samt för villkoren för professionellt yrkesutövande.

    Hälso- och sjukvården berör alla och debattera ständigt. De senaste åren har debatten i stor utsträckning handlat om var gränsen ska gå mellan offentligt och privat. Ska privata aktörer äga akutsjukhus? I vilken utsträckning kommer vi att behöva privata vårdförsäkringar? Hur ser samarbetet mellan läkemedelsindustrin och vården ut? Men debatten handlar också om i vilken utsträckning andra t ex media, patienter och intresseorganisationer ska ha insyn i vårdens praktik och professionernas utövande. Ytterligare en aspekt som diskuteras i boken är vilka intressen som betraktas som offentliga respektive privata. Kapitlen visar att det i det vardagliga arbetet i hälso- och sjukvården samtidigt pågår både motstridiga och komplementära processer, vilka kan delas in i flera dimensioner av offentlig toch privat. Sammantagt pekar dessa processer mot en utveckling som tenderar att fragmentera hälso- och sjukvården. I denna bok ger femton forskare från olika discipliner inblick i detta spännande gränsland mellan offentligt och privat i vården.

    In the context of more and more project-based research funding, commercialization and economic growth have increasingly become rationalized concepts that are used to demonstrate the centrality of science for societal development and prosperity. Following the world society tradition of organizational institutionalism, this paper probes the potential limits of the spread of such rationalized concepts among different types of research funders. Our comparative approach is particularly designed to study the role and position of nonprofit research funders (NPF), a comparison that is relevant as NPF could potentially be shielded from such rationalized pressures given their lack of profit gaining motives. By making a qualitative interview-based investigation we are able to describe how research funders rationalize their contributions to society at large, as well as their obligations to the researchers they fund. Four types of research funders are comparedindependently wealthy philanthropists, fundraising dependent nonprofits, public agencies, and industry. We find that NPF, and especially philanthropists, are the least commercially geared type of funder, but that philanthropists also express least obligations to researchers funded. This is in sharp contrast to public research funders who, even more than industry, employ commercially geared rationalizations. We also find that both public and corporate funders express obligations to the researchers they fund. Our results indicate that there are limits to the spread of commercially tinted rationalizations among NPF, but that this does not necessarily mean an increased sense of obligations to the researchers funded, and by extension to the integrity of scientific pursuit.
