    Abstract [en]

    Purpose

    Being an informal caregiver of an individual with head and neck cancer can be demanding. Still, informal caregivers can provide valuable support to patients throughout the disease trajectory. The aim of this study was to explore informal caregivers’ views on their challenges and needs in attaining high preparedness for caregiving.

    Methods

    Fifteen informal caregivers of individuals with head- and neck cancer participated in a focus group discussion or an individual interview. Thematic analysis utilizing an inductive approach was performed.

    Results

    The results describe the challenges that informal caregivers to individuals with head and neck cancer perceive and their needs for support in preparedness for caregiving. Three main themes were found: Challenges of being an informal caregiver, Transformation in life and Informal caregiver’ needs of support and sharing care.

    Conclusion

    This study contributes to the understanding of the challenges for informal caregivers to individuals with head and neck cancer in increasing preparedness for caregiving. To improve preparedness for caregiving, informal caregivers need education, information and support regarding physical, psychological and social issues for individuals with head and neck cancer.

    Abstract [en]

    It is strongly recommended that randomised controlled trials are preceded with an exploration of the needs of the target population and feasibility testing of the intervention. The present study protocol is set out to describe these steps in the development of a complex intervention.

    The past decades’ transition of care from inpatient to outpatient settings has increased the complexity of caregivers’ responsibilities, which they may not be prepared for. There is a need to support informal caregivers (ICs) to prepare them for caregiving and decrease the caregiver burden. The main aim of this study is to describe the development of an internet-based intervention (Carer eSupport) to improve ICs’ ability to support individuals with head and neck cancer and to describe the testing of the feasibility and acceptability of Carer eSupport.

    Methods and analysis

    This is a multicentre study involving the ear, nose and throat clinics and the oncology and radiotherapy clinics at three university hospitals. The study protocol comprises two phases, development and feasibility testing, using the Medical Research Council framework for developing a complex intervention. Carer eSupport will be based on the results from focus group discussions with ICs and healthcare professionals (planned for n=6–8 in respective groups) and scientific evidence, the Social Cognitive Theory and the Theory of Acceptance and Use of Technology. The feasibility testing will include 30 ICs who will have access to Carer eSupport for 1 month. The feasibility testing will be evaluated with a mixed-method design.

    Ethics and dissemination

    All procedures have been approved by the Ethics Committee at Uppsala University (Dnr: 2020-04650). Informed consent will be obtained before enrolment of patients, their ICs and healthcare staff. The feasibility testing is registered at Clinicaltrials.gov (Identifier: NCT05028452). Findings will be disseminated in peer-reviewed journal publications.

    Abstract [en]

    This course will introduce attendees to the challenges and benefits of integrating User-Centred Design (UCD) methods into the Google Design Sprint (GDS) process. The course will introduce this new User-Centred Design Sprint process, and participants will practice selected methods from the process during the course. By the end of the course, participants will know why, when and how to use User-Centred Design Sprint. Delivered by experienced members of IFIP TC13, this course will appeal to researchers and developers working in the early stages of designing software products.

    Under hösten 2011 har en förstudie genomförts för att kartlägga och analysera dagens processer för utveckling, anskaffande, införande och utvärdering av administrativa IT-system vid Uppsala universitet (UU). Förstudien, Införande av verksamhetsstödjande IT-system. Problem, effekter och nytta, har genomförts av forskare vid avdelningen för människa-datorinteraktion (MDI) vid institutionen för informationsteknologi, UU. Förstudien har gjorts på uppdrag av universitetsdirektör Ann Fust och kontaktperson har varit Britta Nyberg, enheten för planering, uppföljning och stöd för verksamhetsutveckling.

    Arbetet har dels bestått av en kartläggning och analys av dagens utvecklings- och införandeprocesser och de problem man upplever i samband med dessa, dels några analyser av hur användare uppfattar de IT-system man idag har.

    Sammanfattningsvis har kartläggningen och analysen visat att det finns en stor förbättringspotential när det gäller processer för verksamhets- och IT-utveckling samt hur användbarhetsaspekter beaktas i utveckling, anskaffande, införande och utvärdering av IT-system. Kartläggningen av processer som används vid UU visar att det inte finns några enhetliga processer som används generellt inom UU, men att det lokalt finns fungerande sådana. Några exempel är förvaltningen som sedan några år arbetar enligt PM3, och avdelningen för IT och inköp som använder sig av ITIL. I de idag pågående IT-projekten varierar arbetssättet mycket. Det finns inga rekommenderade processer eller arbetssätt utan projektledaren lägger upp arbetet baserat på egna erfarenheter.

    Inom UU arbetar man generellt inte med användarcentrerade processer, och användbarhetsaspekter är inte prioriterade. Inom vissa projekt och i viss förvaltning förekommer en del arbete med fokus på användbarhet. Referensgrupper används som en form av användarmedverkan, men med olika mål och syften. Införandeprocesser upplevs generellt som problematiska inom UU och här finns påtagliga problem. Den upplevda användbarheten i de administrativa IT-systemen varierar, men generellt anser förstudiens intervjupersoner att det finns stor förbättringspotential.

    Fallstudien av Raindanceportalen visar att olika användargrupper upplever systemet på helt olika sätt. De som har utbildning i Raindance (ekonomiadministratörer) och använder det i sina kärnuppgifter upplever att systemet i ganska stor utsträckning stöder arbetet. Övriga användare som använder systemet mer sällan och som administrativt stödsystem har klart större problem med systemet.

    I en avslutande diskussion ger vi en sammanställning av de observerade problemen, ger förslag på vad som fordras för att utveckla kunskaper kring problem och möjligheter och för att införa förbättrade processer, samt för att   på sikt   få bättre och effektivare administrativa IT-system inom UU.

    Deployment of IT is little discussed in research literature, despite the fact that a successful deployment encompasses complexities and difficulties well worth investigating. The deployment phase of IT systems can be a make-or-break moment for usefulness of the system due to sociotechnical factors. We argue that there is a gap between the technological artefact produced and the social requirements that govern how well the system will fit in the organisation. Hence, in this work in progress paper we present a framework (the SOT framework) for analysing deployment of IT from a sociotechnical perspective.

    Abstract [en]

    How can students be included as critical stakeholders in the systems and services provided by a university? To address the whole student experience, we engaged students and employees at a large Swedish university in a vision seminar process to elicit how these groups envisioned an ideal future university, and the necessary changes to technology and organisational structures required to achieve this ideal version. The process entailed six four-hour workshops with four groups consisting of six participants each. A survey instrument was used to follow up on the participants' experiences of participating in the vision seminar process and their thoughts on the future of the university. The results show that the participating students were more positive compared to the university employees. The students envisioned harmonized interdepartmental systems, seamlessly integrating a variety of services into one university provided solution. The employees envisioned their future work as flexible, enabled by technology providing excellent support without hindering pedagogical and organisational development. Using technological frames, these visions of the future are identified, analysed and discussed in relation to the quality of university education and a holistic view on students' university experience. Finally we discuss the broader implications of the visions on the future of university education.

    Abstract [en]

    It can be hard to understand the context in which a software system is used and how the whole work environment affects the usage of a particular system. In this project, we modified the Persona method to include aspects of the digital work environment and the context to extend the understanding of these aspects during software development. The modified version of the Persona method is based on the traditional Persona method, on theories on healthy work and research on the digital work environment. The objective of proposing the modifications of the method is to give software developers more insights into the complexity of the digital work environment. University students tried the modified Contextual Persona method in a user-centred software development course. Students worked on designing a new system for 12 weeks using the Contextual Persona method during the fourth week of software development. The students gave feedback on what positive and negative aspects they experienced while using the modified Contextual Persona method and their thoughts on how the method could be improved. In the paper, we analyse reports from 30 students and summarise the feedback gathered. We conclude by summarising the possible improvements to the method based on our findings.

    The work environment for nurses is challenging, and even though new technology has major advantages, it has often also caused new stressors and problems for nurses. When Patient Accessible Electronic Health Records (PAEHR) was introduced in Sweden, research showed that nurses were worried about possible negative effects for patients as well as on their work. However, to this date there are very few follow up studies on the more long term effects of PAEHR, despite research pointing to the fact that reactions after long term use might differ from initial experiences. In this paper we present an interview study to fill this research gap. An analysis based on interviews with physicians and nurses in Oncology care reveals three areas where nurses' work is experienced to have been affected: 1) nurses receive more questions from patients after PAEHR has been introduced, 2) nurses have changed their documentation practices and 3) the log list functionality has made nurses feel questioned. Finally, these results are discussed in relation to nurses' work environment from a sociotechnical and gender perspective.

    Patient accessible electronic health records have been launched in many countries, and generally, health-care professionals have had strong initial concerns related to the areas patient contact, documentation practices and quality of care. Especially, oncology care was discussed in media when launching patient accessible electronic health records in Sweden. However, few studies have investigated clinician-perceived effects several years after the launch. A survey covering these areas, as well as supposed effects for patients, was distributed to oncology health-care professionals 6 years after the launch of patient accessible electronic health records and answered by N = 176. Results show that patient accessible electronic health records have had small effects within the covered areas, and that the area most affected was documentation practices. Very few significant differences could be found between physicians and nurses. A comparison with results from interviews and surveys conducted shortly after the launch of patient accessible electronic health records clearly indicates that the experienced negative effects are not as big as originally feared.

    Background: Internationally, there is a movement toward providing patients a Web-based access to their electronic health records (EHRs). In Sweden, Region Uppsala was the first to introduce patient-accessible EHRs (PAEHRs) in 2012. By the summer of 2016, 17 of 21 county councils had given citizens Web-based access to their medical information. Studies on the effect of PAEHRs on the work environment of health care professionals have been conducted, but up until now, few extensive studies have been conducted regarding patients' experiences of using PAEHRs in Sweden or Europe, more generally.

    Objective: The objective of our study was to investigate patients' experiences of accessing their EHRs through the Swedish national patient portal. In this study, we have focused on describing user characteristics, usage, and attitudes toward the system.

    Methods: A national patient survey was designed, based on previous interview and survey studies with patients and health care professionals. Data were collected during a 5-month period in 2016. The survey was made available through the PAEHR system, called Journalen, in Sweden. The total number of patients that logged in and could access the survey during the study period was 423,141. In addition to descriptive statistics reporting response frequencies on Likert scale questions, Mann-Whitney tests, Kruskal-Wallis tests, and chi-square tests were used to compare answers between different county councils as well as between respondents working in health care and all other respondents.

    Results: Overall, 2587 users completed the survey with a response rate of 0.61% (2587/423,141). Two participants were excluded from the analysis because they had only received care in a county council that did not yet show any information in Journalen. The results showed that 62.97% (1629/2587) of respondents were women and 39.81% (1030/2587) were working or had been working in health care. In addition, 72.08% (1794/2489) of respondents used Journalen about once a month, and the main reason for use was to gain an overview of one's health status. Furthermore, respondents reported that lab results were the most important information for them to access; 68.41% (1737/2539) of respondents wanted access to new information within a day, and 96.58% (2454/2541) of users reported that they are positive toward Journalen.

    Conclusions: In this study, respondents provided several important reasons for why they use Journalen and why it is important for them to be able to access information in this way-several related to patient empowerment, involvement, and security. Considering the overall positive attitude, PAEHRs seem to fill important needs for patients.

    IntroductionThis study investigates factors related to long-term and short-term adoption of video consultations (VCs) and reasons for discontinuing use among primary care patients.MethodsA sample of primary care patients using VCs with healthcare providers were invited to take a survey in a cross-sectional study. Participants were asked about their intention to continue to have video consultations in the future, and those indicating no intention to use VCs in the future (short-term adopters) were asked about their reasons for this. Prevalence and statistical differences between long-term and short-term adopters were investigated.ResultsThere were several statistically significant differences between long-term and short-term adopters (76% vs. 24%). Long-term adopters consisted of more middle-aged individuals (35?54 years) and the majority worked full-time (56%). They had more positive opinions of VCs and used VCs and video meetings for other purposes to a larger extent. They chose VCs because of the lack of time to go to the healthcare centre and because their provider offered them. The most common reason for discontinuing use was a preference for face-to-face consultations, with the youngest age group (16?34 years) reporting this to a larger extent.DiscussionYounger and older age groups may be less likely to continue the use of VCs, potentially preserving the digital divide. Additionally, disparities in using similar technologies might contribute to the digital divide. Moreover, convenience, positive opinions of VCs, and experience with VCs were related to long-term adoption. Further studies are needed to explore non-use, age?s influence, and address usability issues.

    For healthcare personnel, the work environment is already challenging, and when eHealth systems are introduced they are often considered to further add to the complexity. This paper investigates the impact of patients' access to their electronic health records on healthcare professionals' work environment in a primary care setting in Sweden. A work environment theory-driven analysis, focusing on perceived demands, control and support, was conducted on 14 semi-structured interviews of different primary care professionals. The professionals expressed a slight increase in demands, loss of control and some increase and decrease of support. This study discusses insights on how patients' access to health records can have an impact on healthcare professionals' work environment.

    This study explores patients’ perspectives on sharing their personal health data, which is traditionally shared through discussions with peers and relatives. However, other possibilities for sharing have emerged through the introduction of online services such as Patient Accessible Electronic Health Records (PAEHR). In this article, we investigate strategies that patients adopt in sharing their PAEHR. Data were collected through a survey with 2587 patients and through 15 semi-structured interviews with cancer patients. Results show that surprisingly few patients share their information, and that older patients and patients with lower educational levels share more frequently. A large majority of patients trust the security of the system when sharing despite the high sensitivity of health information. Finally, we discuss the design implications addressing identified problems when sharing PAEHR, as well as security and privacy issues connected to sharing.

    Education can be seen as a preparation for a future profession, where some educational programs very clearly prepare their students for a certain profession, e.g. plumber, nurse and architect. The possible professions for students following education programs in computing is quite varied and thus difficult to cater for, but to educate towards a professional life is still a stated goal in most higher education settings. We argue that this goal is typically not even closely reached and provide an analysis indicating factors explaining this situation. The analysis is based on the concept of professional identity. In earlier work [1] a framework with which to reason about student interactions with the regulatory structure of higher education and teachers was developed. In that paper we developed a compound model which not only relates these players to one another, but also provides approaches to reasoning about misalignments which arise when students and teachers approach their shared learning context from different perspectives. This framework is in this paper applied to address different aspects of professional identity with the intent of bringing forth deeper insights into challenges with educating towards professions. This issue is highly complex and the framework provides a structure that is beneficial for analysing different aspects in a more holistic manner.

    Currently, health research increasingly involves diverse groups of stakeholders. Such collaborations occur on various levels, where in some cases, stakeholders become co-researchers. However, these stakeholders (e.g. patients) are not always trained in the necessary research skills. On the other hand, researchers are not trained to collaborate with stakeholders. While there is agreement that skill training should be offered as an integral part of participatory health, so far knowledge is lacking on what such a training should look like. This workshop aims to collect experiences from those who have either previously implemented stakeholder skill training or have experiences where stakeholder skill training was lacking but would have been beneficial, and facilitate the exchange of ideas between participants who are interested in the topic. The outcomes will be communicated in ways that are accessible for academic and non-academic stakeholders and will provide them with guidance for conducting stakeholder skill training.

    Abstract [en]

    The aim of this special session is to connect researchers interested in computing and engineering education for sustainability. We will explore the use of care and care ethics as a theoretical perspective to develop sustainability education. Theoretical discussions in environmental and sustainability education (ESE) research and feminist research will be introduced to develop an understanding of care for education. Those theories will be illustrated and motivated based on concrete examples in computing and computing education. The participants get to choose among four different topics of discussion in the session, 1) the role of education to prepare for care, 2) theoretical discussions of care as a concept to develop education and education research, 3) pedagogical methods to foster care, 4) care and responsibility in the curriculum. The outcome of this session is two-fold: The participants will gain new ways of conceiving education for human and planetary well-being and they will get to know researchers and educational developers with an interest in and experiences with sustainability education.

    The learning agreement has great potential as a learning intervention that supports learners to take ownership of their learning and to develop in ways that are meaningful to them. It is useful to educate a diverse student cohort. In a project course as the one investigated in this study, the learning agreement intervention can be beneficial to make use of the different experiences and competencies that a diverse student group brings to the table. In practice however, the learning agreement invention is uncommon in education and not well understood. As it is used in the present study, it is essentially a document in which the students describe competencies that they want to develop, as well as how they will develop and assess those competencies. This study has investigated the learning agreement intervention as it was conducted in an open-ended group project, in particular a workshop to improve the quality and usefulness of the students' learning agreements. Two versions of learning agreements, the one before the workshop and the one after, from 19 students were analysed, as well as semi-structured interview data with the students. We find that many learning agreements are of little use, even after the workshop. A qualitative thematic analysis suggests that the students experience the learning agreement and workshop as useful but that they still struggle with the learning agreement, particularly with describing activities to develop and assess their learning. We derive ideas for how to improve the learning agreement intervention, e.g. by integrating it more with the project work.

    Restructuring Teaching/Learning and Assessment Activities in an Online Course to Ensure Constructive Alignment2024Conference paper (Refereed)
    Abstract [en]

    This study explores the importance of constructive alignment in enhancing student engagement and learning outcomes in online courses, focusing on an online Human-Computer Interaction (HCI) course. We interviewed teaching assistants and reviewed the course feedback from students to identify challenges faced by teaching assistants and students, including unclear learning objectives, lack of engaging content, and insufficient assessment diversity. Drawing on feedback from both groups, we propose restructuring the course based on constructive alignment principles. Recommendations include incorporating recorded lectures, podcasts, seminars, and group discussions; restructuring assignments into smaller groups with clear guidelines and self-introduction videos; and diversifying assessment methods with design projects, final examinations, and quizzes. Additionally, we emphasize the importance of timely and clear feedback, explicit assignment instructions, and well-defined course guidelines. Implementing these recommendations aims to address existing challenges, enhance student engagement, and ensure clear alignment between teaching, learning, and assessment in the online HCI course.

    Being an informal caregiver is not easy, and might cause physical and psychosocial burden, especially in the long run. However, the formal health care system has little support for informal caregivers who experience abandonment and lack information. Mobile health can potentially be an efficient and cost-effective way of supporting informal caregivers. However, research has shown that mHealth systems often have problems with usability, and people do not use the systems for more than a short period. Therefore, this paper explores the design of an mHealth app using Persuasive Design, an established design framework. This paper presents the design of the first version of the e-coaching application using the persuasive design framework and unmet needs of informal caregivers from the literature. This prototype version will be updated based on interview data from informal caregivers in Sweden.

    Background: Informal caregivers are vital in caring for their family and friends at home who may have illnesses or disabilities. In particular, the demands for caregiving can be even more challenging for those with limited resources, support systems, and language barriers, such as immigrant informal caregivers. They face complex challenges in providing care for their relatives. These challenges can be related to sociocultural diversity, language barriers, and health care system navigation. Acknowledging the global context of the increasing number of immigrants is essential in designing inclusive mobile health apps.

    Objective: This study aims to investigate the needs of immigrant informal caregivers in Sweden and discuss the application of the Persuasive System Design Model (PSDM) to develop an e-coaching prototype. By addressing the unique challenges faced by immigrant informal caregivers, this study will contribute to the development of more effective and inclusive mobile health apps.

    Methods: The participants were considered immigrants and included in the study if they and their parents were born outside of Sweden. Through various channels, such as the National Association of Relatives, rehabilitation departments at municipalities, and immigrant groups, we recruited 13 immigrant informal caregivers. These immigrant informal caregivers were primarily women aged 18 to 40 years. Most participants belonged to the Middle Eastern region whereas some were from North Africa. However, all of them spoke Arabic. We used semistructured interviews to gather data from the participants in Arabic, which were translated into English. Data were analyzed using thematic analysis and discussed in relation to the extended PSDM. The needs of the caregivers were compared with the description of persuasive design principles, and a design principle was chosen based on the match. The PSDM was extended if the need description did not match any principles. Several brainstorming and prototyping sessions were conducted to design the mobile e-coaching app.

    Results: Immigrant informal caregivers have various needs in their caregiving role. They reported a need for training on the illness and future caregiving needs, assistance with understanding the Swedish language and culture, and help with accessing internet-based information and services. They also required recognition and appreciation for their efforts, additional informal support, and easy access to health care services, which can be important for their mental health. The PSDM was adapted to the informal caregiving context by adding “facilitating conditions” and “verbal encouragement” as additional persuasive design principles. This study also presents the subsequent mobile e-coaching app for immigrant informal caregivers in Sweden.

    Conclusions: This study revealed important immigrant informal caregivers’ needs based on which design suggestions for a mobile e-coaching app were presented. We also proposed an adapted PSDM, for the informal caregiving context. The adapted PSDM can be further used to design digital interventions for caregiving.

    Objective: Informal caregivers such as relatives or close friends of patients are essential for caregiving at home. However, caregiving is a complex experience that may affect the caregivers’ well-being. Therefore, there is a need to provide support for caregivers, which we address in this article by proposing design suggestions for an e-coaching application. This study identifies the unmet needs of caregivers in Sweden and provides design suggestions for an e-coaching application using the persuasive system design (PSD) model. The PSD model offers a systematic approach to designing IT interventions.

    Methods: A qualitative research design was used, and semi-structured interviews were conducted with 13 informal caregivers from different municipalities in Sweden. A thematic analysis was performed to analyze the data. The PSD model was used to map the needs emerging from this analysis to propose design suggestions for an e-coaching application for caregivers.

    Results: Six needs were identified, and based on them, we proposed design suggestions for an e-coaching application using the PSD model. These unmet needs are monitoring and guidance, assistance to avail formal care services, access to practical information without being overwhelmed, feeling of community, access to informal support, and grief acceptance. The last two needs could not be mapped using the existing PSD model, resulting in an extended PSD model.

    Conclusion: This study revealed the important needs of informal caregivers based on which design suggestions for an e-coaching application were presented. We also proposed an adapted PSD model. This adapted PSD model can be further used for designing digital interventions in caregiving.

    Background: A growing number of informal caregivers in many countries are important for effective functioning of health care in our society. Hence, they must receive the required support and services to continue providing care. IT applications can be used to support informal caregivers in their caregiving activities. However, evidence-informed guidelines for developing such IT applications and their evaluation are scarce. Hence, this scoping review can aid researchers and designers by providing design recommendations for IT apps for caregivers and potentially improve the design of IT applications for caregivers to meet their needs better.

    Objective: This study aims to describe the proposal for a scoping review of current practices and recommendations in designing and evaluating IT applications for informal caregivers. The scoping review will also discuss the opportunities and challenges in designing these IT applications.

    Methods: We will use a 5-step scoping review methodology to map relevant literature published as follows: (1) identify the research question, (2) identify relevant studies, (3) select relevant studies for review, (4) chart the data from selected literature, and (5) summarize and report results. A structured search will be conducted in PubMed, Scopus, IEEE digital library, Web of Science, and ACM Digital Library databases. In addition, hand searches of reference lists and keyword searches in Google Scholar will also be conducted. Inclusion criteria will be researched (journal and conference) articles focused on IT applications designed for use by informal caregivers and study type to be qualitative studies. Two reviewers will independently identify articles for review and extract data. Conflicts will be discussed, and a third reviewer will be consulted if a consensus cannot be reached. These data will be analyzed using thematic analysis.

    Results: The results from this scoping review will be presented in a narrative form, and additional data on study characteristics will be presented in diagrams or tabular format. Uppsala University first initiated this scoping review protocol in December 2021 as part of the European Union–funded project ENTWINE. This work was also supported by the Swedish Research Council and the Swedish Cancer Society. The results will be presented in August 2023 and will be disseminated through a report to the European Union and a peer-reviewed journal publication. In addition, the team plans to share its findings on various public platforms, including social media, blog posts, and relevant conferences and workshops.

    Conclusions: This is, to our knowledge, the first study to map the literature on the design and evaluation of IT applications for informal caregivers. The scoping review’s results will detail the requirements, design suggestions, and user preferences, as well as the usability criteria and features of IT applications for informal caregivers. A mapping of studies could inform the design and implementation of future IT applications for informal caregivers.
