    Abstract [sv]

    Den här uppsatsen presenterar en enkätundersökning som syftar till att ta reda på amerikanska skandinavistikstudenters attityder till Sverige och det svenska språket. Det empiriska materialet består av enkätsvaren från 44 informanter som alla studerar svenska och/eller skandinavistik på eftergymnasial nivå på ett college i Minnesota, USA. Svaren har analyserats med kvantitativ och kvalitativ ansats för att sättas i relation till Hansens teori om olika invandrargenerationers attityder till sin kulturella bakgrund. Resultatet visar att det finns generationsskillnader men att graden av identifikation som svensk i regel sammanfaller med att vara invandrare från Sverige, oavsett generation, men att det inte är någon av dessa faktorer som är mest avgörande för hur positiv attityd man har mot det svenska språket. En mer avgörande faktor skulle i så fall vara huruvida man i sina studier valt att specialisera sig på ämnet/ämnena svenska och/eller skandinavistik. Resultatet visar också att medan termen Swedish-American kan väcka såväl positiva, negativa som neutrala konnotationer, skapar och experimenterar informanterna själva med sin svenska identitet vilket det förefaller finnas stort utrymme för eftersom Sverige och svenskan har en sammantaget hög status i det geografiska området för undersökningen. Det svenska språket i denna skapandeprocess kan fungera både som ett ändamål i sig och som ett medel för att nå något annat.

    Global investigations show that one in three women has experienced intimate partner violence at some point during her lifetime. Intimate partner violence includes physical, psychological and sexual violence or coercion as well as limitations of freedom. Intimate partner violence is a societal issue as well as one for the woman exposed. It is clear that many women are affected, and that healthcare and it´s providers share an important role in disclosure, treatment and support for the women.

    Studiens syfte är att undersöka lärares användning av namn och könande ord i tilltalet gentemot elever samt lärares medvetenhet och resonemang gällande tilltal. Könande ord vilket beskrivs senare i arbetet syftar på ord som tillskriver någon ett kön. Tidigare forskning visar att elever i den svenska skolan ges olika möjligheter att tala och bli tilltalade beroende på vilket biologiskt kön de antas tillhöra. I denna studie undersöks området genom observationer av och intervjuer med tre lärare. Lärarna arbetar i förskoleklass till årskurs tre på två olika skolor i Uppsala kommun. Studien utgår från teorier om genus och identitetsskapande med fokus på interaktion. De tre fallen analyseras och likheter och skillnader mellan de deltagande lärarnas tilltal och resonemang identifieras och presenteras. Studien visar att pojknamn används i högre utsträckning än flicknamn i lärares tilltal till elever, samt att könande ord förekommer i mycket liten utsträckning. Vidare framgår att de deltagande lärarna resonerar mycket kring tilltal till eleverna, men endast en av tre resonerar kring tilltal i förhållande till genus. Genom att genomföra detta självständiga arbete önskar vi att uppmärksamma hur lärares tilltal till elever och resonemang kring detta yttrar sig i praktiken. Denna studie kan ses som ett verktyg i processen mot att bli mer genusmedveten i sin yrkesutövning.

    Denna studie undersöker hur narkomanen framställts på de opinionsbildande sidorna hos fyra svenska rikstäckande dagstidningar: Dagens Nyheter, Aftonbladet, Expressen och Svenska Dagbladet. Studien behandlar åren 1969, 1979 och 1989 i syfte att analysera hur narkomanen framställts under den tid då den moderna svenska narkotikapolitiken formats. Under mitten av 1960-talet kom narkotikan att etableras som ett offentligt och socialt problem vilket ansågs kräva avsevärda samhälleliga insatser. Sedan dess har en intensiv debatt bedrivits för att försöka förstå orsakerna bakom att individer använder narkotiska preparat, samt vilka åtgärder som bör sättas in för att bekämpa drogmissbruket. Den uttalade målsättningen för den svenska drogpolitiken är ett narkotikafritt samhälle, vilket lett till att den restriktiva kontrollpolitiken fått stora befogenheter, bland annat genom kontinuerligt förhöjda straffsatser. Trots att politiken utvecklats efter en intensifierat repressiv linje, har denna undersökning visat att det inom den mediala drogdebatten konstant funnits varierande och skiftande framställningar av narkomanen. Jag har även visat på att gränsen mellan det normala och det avvikande förskjutits mellan åren. De två vanligaste aspekterna i framställningen av narkomanen är å ena sidan som förslavad under drogerna med tillhörande oförmåga att se till sitt eget bästa, å andra sidan som ett offer för antingen en felaktigt förd politik eller samhälleliga brister.

    Abstract [en]

    Objective: Children with the severe forms of osteogenesis imperfecta have in several studies been treated with intravenous pamidronate, but there are only few reports of the effect of early treatment.

    Aim: To evaluate the effect of treatment started in infancy.

    Methods: In a prospective observational study, with a historic control group, intravenous disodium pamidronate (APD) was given as monthly infusions to 11 children with osteogenesis imperfecta aged 3–13 (median 3.6) months, who had severe osteogenesis imperfecta with congenital bowing of the femora and vertebral compression fractures.

    Results: During treatment of children aged between 3 and 6 (median 4.5) years, dual-energy x ray absorptiometry measurements of the lumbar spine showed a gradual increase in bone density. Bone metabolism parameters in serum (alkaline phosphatase, osteocalcin, procollagen 1 carboxy-terminal peptide, collagen 1 teleopeptide) and in urine (deoxypyridinoline) indicated a decrease in bone turnover. An improvement of mobility was seen and at the latest recording, at the age of 3.3–6.5 (median 4.8) years, the children could all walk. Vertebral remodelling was seen, with increased vertebral height, and no child developed scoliosis, kyphosis or basilar impression. All children required femoral intramedullar rods for fractures, and five needed tibial rodding for extreme curvatures that prevented functional standing and walking. No adverse effects were seen on growth, fracture healing or blood chemistry.

    Conclusions: APD is an efficient symptomatic treatment for infants with severe osteogenesis imperfecta, but additional orthopaedic surgery is often needed. Early treatment may prevent scoliosis and basilar impression. Long-term follow-up is important.

    PURPOSE:

    To describe the pathological features and assess the diagnostic information of different MR sequences in patients with primary, secondary, and mixed (phlebo-, lipophlebo-, or lipolymphedema) forms of lymphedema of the lower leg.

    MATERIAL AND METHODS:

    In 26 patients with clinical diagnoses of primary (n=10), pure secondary (n=4), mixed (n=9) and combined secondary and mixed forms of lymphedema (n=3), MR imaging was performed with coronal and axial T1 SE, T2 TSE, fat-suppressed (SPIR) T2 sequences and axial T1 SE after i.v. injection of Gd-DTPA.

    RESULTS:

    In 24 patients there was a honeycomb pattern in the subcutis with a signal intensity corresponding to fluid (n=11), fibrosis (n=3), or both (n=10). Five patients with primary lymphedema showed subfascial fluid accumulation. Dermal edema was noted in 23 patients. Fat or edema components in the muscles were mostly seen in patients with phlebolymphedema. The honeycomb pattern was best seen on coronal T1 images, and fluid accumulations on axial SPIR-T2 images. Fibrosis was only assessible from the T2 TSE sequence. Gd-DTPA did not improve the diagnostic information.

    CONCLUSION:

    For evaluation of lymphedema and its mixed forms, an axial T2-weighted SPIR sequence in conjunction with a coronal T1 SE sequence are sufficient.

    Abstract [en]

    Computed tomography-guided transsternal biopsy was successful in 10 anterior mediastinal masses in 10 patients, with use of a coaxial length-matched bone biopsy system comprising an outer cannula and an inner eccentric drill bit. No complications occurred in nine of 10 biopsies (eight performed with an automatic cutting needle, two with a fine needle), with less discomfort than was caused by injection of anesthetic.

    Abstract [en]

    Twenty-eight consecutive CT (n = 23) or ultrasonographically (n = 5) guided biopsy procedures were performed on musculoskeletal lytic lesions covered (n = 13) or not covered (n = 15) with intact bone. Specimens were obtained by means of Biopty techniques (n = 27), i.e. Biopty and Monopty instruments, through different cannulas with normal or shortened needle-throws. Four out of 5 bone penetrations were successful with an Ostycut needle, and all 8 bone penetrations by a coaxial bone biopsy system with an eccentric drill. The eccentric drill makes a hole in the cortical bone larger than the diameter of the outer cannula of this system, making it easy to anchor the cannula and then coaxially insert a Biopty-Gun needle for example. The overall histopathological diagnostic accuracy of the Biopty techniques was 25/27 (92.6%).

    Abstract [en]

    The increasing levels of the greenhouse gas carbon dioxide (CO2) in the Earths atmosphere, caused by human release of CO2, has made it desirable to understand the factors determining the CO2-variation because of CO2’s warming effect on the Earths temperature which will change the premises of all life on earth.

    The purpose of this investigation is to understand the effects of the largest factors of influence on the CO2-concentration - like sea, vegetation and anthropogenic outlets - in the Baltic Sea region, and possible surprises from the results. To be able to do this only from CO2-measurements some assumptions have to be done as starting point. Such are that, besides from the yearly trend of the CO2-concentration and the variation of oceanic influence, monthly variation only is caused by vegetation and that the yearly offset in CO2-levels only is affected by anthropogenic outlets. These factors are together called the local season and will be used for evaluation of the CO2-values for each site. This analysis is done for eight sites surrounding the Baltic Sea region and is compared with results from the site of Östergarnsholm, an island in the Baltic Sea east of Gotland.

    The results show that stations with high vegetational influence has high amplitudes for the local season compared to sites more influenced by sea. This also makes the amplitude to be connected with latitude since sites with longer growing season is surrounded by higher density of vegetation. The minimum for the local season is also dependent on the growing season, since it occurs when the vegetational consumption is largest. Peaks in the local season can be seen in connection with the maximum decay of the natural vegetation in the early winter months, and with the planting and harvest season for agricultural land. Considering the effect from anthropogenic influence a clear connection in the offset of the local season can be seen, with higher offsets for sites of higher anthropogenic influence and vice versa. Anthropogenic influence also seems to give raised values in summer for the local season, indicating that the variation of the local season cannot be simply connected to only vegetational influence. For variability, higher values in the summer months are seen for the anthropogenic sites, while in winter the variability is more similar for all sites. This might be connected with a higher degree of local influence during summers, which for anthropogenic stations leads to high variability due to inhomogenous surroundings.

    For Östergarnsholm we get higher amplitude for the local season than expected, this is partly due to unrepresentatively high amplitudes for the seasons used, but also probably to some degree of underestimation of the vegetational influence. Due to correction of the offset it was not possible to draw any conclusions from this factor, but rather give suggestions of what the correction should be. When analysing the local season for different source areas by WD-classification we see the surprising property that the sector that should be most influenced by land, due to higher values in summer, has a lower amplitude than the sector most influenced by sea. Since it was suggested that anthropogenic influence gives raised values in summer this was suggested as an explanation.

    Abstract [en]

    The purpose of this essay is to examine how Christians can and should speak about God if they take feminist criticism seriously. This purpose concerns two problems: the first starts with the proposition that God is essentially different from humans and things and at the fact that the language that we use to describe and speak about God is a human language. God is infinite, incorporeal, and timeless, while the human language normally is used to apply to finite, corporeal, and temporal things. How – if ever – can this language apply to God? To examine this problem further, I present four different theories of religious language.

    The second problem is actualized by feminists who criticize the standard within the Christian tradition to characterize God in predominantly masculine terms. Feminists have criticised religious language for being oppressive in several ways, and particularly to establish and maintain hierarchical structures in which women are subordinated men. In this essay I present and discuss feminist criticism of religious language and then distinguish four different strategies for feminists. I further examine three of these strategies, represented by Sallie McFague, Gail Ramshaw and Janet Soskice, dealing with the problem of religious language within the Christian tradition. In all three of these feminist strategies metaphors are found to be of great importance.

    Finally, I promote Soskice metaphor theory combined with an apophatic theory of language. Soskice stresses the importance of anthropomorphous metaphors and offer the possibility of using both male and female images when speaking about God. This strategy positively handles the proposition of God as essentially different but makes it possible for believers to refer to God (through metaphors). This is also the preferable strategy in line with feminist criticism.

    Uppsatsen syfte är att undersöka mötet mellan feministisk religionsfilosofi och frågan om religiös mångfald. Utgångspunkten för uppsatsen är religionsfilosofiska texter av Pamela Sue Anderson och Grace Jantzen och dessa texter får här representera och ligga till grund för en feministisk religionsfilosofi. Som representant för det filosofiska samtalet om religiös mångfald använder jag mig av Mikael Stenmarks översikt över den typologi som präglar det filosofiska samtalet om religiös mångfald. Uppsatsens frågeställning lyder: Vad kan en feministisk religionsfilosofi som tar sin utgångspunkt i Grace Jantzens och Pamela Sue Andersons religionsfilosofiska texter bidra med till det filosofiska samtalet om religiös mångfald?

    Genom att läsa Andersons och Jantzens litteratur svarar jag för det första på hur en feministisk religionsfilosofi som tar sin utgångspunkt i Andersons och Jantzens texter skulle kunna se ut. För det andra undersöker jag hur en feministisk religionsfilosofi förhåller sig till det filosofiska samtalets typologi (exklusivism, inklusivism och pluralism) vad gäller religiös mångfald. Slutligen svarar jag på vad en feministisk religionsfilosofi kan bidra med till det filosofiska samtalet om religiös mångfald. Ett första bidrag kan innebära en medvetenhet och erfarenhet om mångfalden som finns ibland oss. Ett andra bidrag kan innebära en diskurskritik och en medvetenhet av att religionen inte går att separera från samhälleliga normer och diskurser. Ett tredje bidrag kan handla om fantasi och föreställningsförmåga som strategi.

    På grund av globalisering, migration och kommunikation som snabbt tar oss över jordklotet uppstår möten mellan människor med olika erfarenheter, traditioner, kulturer och religioner. Detta innebär nya utmaningar som inte bör lösas genom att dela in människor i påstått homogena grupper och försöka lösa konflikter och oenighet mellan dessa grupper. Den feministiska religionsfilosofin med dess utmaningar - att granska maktstrukturer, söka alternativa bilder och våga göra nya tolkningar av religioners funktioner och betydelser i ljuset av feministisk teori - riktar blicken mot mellanmänskliga relationer, oavsett religionstillhörighet.

    Denna uppsats behandlar övergången mellan förskola och förskoleklass med fokus på samverkan och dokumentation av barns språkutveckling utifrån ett läroplansteoretiskt perspektiv.  Uppsatsens undersökning består av två delar. I den ena undersöks hur övergångar, samverkan och dokumentation skrivs fram i läroplanen, dvs. formuleringen av dessa fenomen. Metoden i denna del består således av en kvalitativ innehållsanalys. I den andra empiridelen undersöks pedagogers upplevelser av samma fenomen genom en webbenkät i syfte att även få en inblick i hur dessa fenomen realiseras i praktiken.

        I analysen av läroplanerna framgår det att verksamheterna ska samverka på ett tillförlitligt sätt med varandra för att främja elevernas progression i ett långsiktigt perspektiv. Här framgår att lärarna inom varje verksamhet har ett gemensamt ansvar för att se till att det skapas goda förutsättningar för eleven att utvecklas och att det skapas möjligheter tillägna sig kunskaper. Genom resultaten i enkätundersökningen visas att det finns svårigheter att realisera det som är formulerat i styrdokumenten om just samverkan. Personal i både förskola och förskoleklass efterfrågar en ökad samverkan för att skapa bättre förutsättningar för eleverna i samband med övergång och även för en gynnsam långsiktig kunskapsutveckling. När det gäller samverkan mellan mottagande förskoleklass, vårdnadshavare och barn tycks det ha fungerat väl. Stor del av respondenterna menar att överlämning om barns språkutveckling har förekommit och några svarade att de anpassat undervisningen beroende på den information de fått vid överlämningen.

    I styrdokumenten poängteras att verksamhetens pedagogiska ansvar och dokumentationskrav åvilar förskollärare/pedagog. Dock framgår det i undersökningen att det saknas direktiv på vad och hur det ska dokumenteras. Enligt respondenterna används olika metoder för att dokumentera barns utveckling. Det framgår att nästan alla är eniga om överlämningens pedagogiska betydelse för barnets fortsatta utveckling.

    Abstract [en]

    Over the next century, one of the largest contributions to sea level rise will come from ice sheets and glaciers calving ice into the ocean1. Factors controlling the rapid and nonlinear variations in calving fluxes are poorly understood, and therefore difficult to include in prognostic climate-forced land-ice models. Here we analyse globally distributed calving data sets from Svalbard, Alaska (USA), Greenland and Antarctica in combination with simulations from a first-principles, particle-based numerical calving model to investigate the size and inter-event time of calving events. We find that calving events triggered by the brittle fracture of glacier ice are governed by the same power-law distributions as avalanches in the canonical Abelian sandpile model2. This similarity suggests that calving termini behave as self-organized critical systems that readily flip between states of sub-critical advance and super-critical retreat in response to changes in climate and geometric conditions. Observations of sudden ice-shelf collapse and tidewater glacier retreat in response to gradual warming of their environment3 are consistent with a system fluctuating around its critical point in response to changing external forcing. We propose that self-organized criticality provides a yet unexplored framework for investigations into calving and projections of sea level rise.

    Abstract [en]

    A particle-based computer simulation model was developed for investigating the dynamics of glaciers. In the model, large ice bodies are made of discrete elastic particles which are bound together by massless elastic beams. These beams can break, which induces brittle behaviour. At loads below fracture, beams may also break and reform with small probabilities to incorporate slowly deforming viscous behaviour in the model. This model has the advantage that it can simulate important physical processes such as ice calving and fracturing in a more realistic way than traditional continuum models. For benchmarking purposes the deformation of an ice block on a slip-free surface was compared to that of a similar block simulated with a Finite Element full-Stokes continuum model. Two simulations were performed: (1) calving of an ice block partially supported in water, similar to a grounded marine glacier terminus, and (2) fracturing of an ice block on an inclined plane of varying basal friction, which could represent transition to fast flow or surging. Despite several approximations, including restriction to two-dimensions and simplified water-ice interaction, the model was able to reproduce the size distributions of the debris observed in calving, which may be approximated by universal scaling laws. On a moderate slope, a large ice block was stable and quiescent as long as there was enough of friction against the substrate. For a critical length of frictional contact, global sliding began, and the model block disintegrated in a manner suggestive of a surging glacier. In this case the fragment size distribution produced was typical of a grinding process.

    Mobile devices such as tablets and smartphones are quite common in this day. The author of this thesis wanted to know how common these mobile devices are in the building industry and how they are used and what potential an app could have. The author thus conducted an investigation on this matter, mainly in the form of interviews.

    Answers given by the interviews were then used as a basis to survey the usage of mobile devices and the usage of different kinds of apps. An investigation of different apps with the potential to be used by professionals in the building industry has been carried through. Apps which manage such things as scanning documents, measuring distances and handle communication. A further more advanced study of a floor plan making app, called MagicPlan has also been done. The equipment that were used were Samsung Galaxy S2, Apple iPad , Samsung Note 10.1 Tablet, Leica Disto, Leica Scanstation C10, Canon Pixma MP270.

    The results of the examination show that the usages of mobile devices are widely spread in the building industry, however, seldom to its full potential. The results also indicates that the tested apps likely can be used by professionals in some cases.

    We have identified a 3' exonuclease in HeLa cell extracts which deadenylates mammalian mRNA and leaves the mRNA body intact after poly(A) removal. Only homopolymeric adenosine tails located at the 3' end were efficiently removed by the exonuclease. The poly(A) removing activity did not require any specific sequences in the mRNA body either for poly(A) removal or for accumulation of the deadenylated mRNA. We conclude that the poly(A) removing activity is a 3' exonuclease since (i) reaction intermediates gradually lose the poly(A) tail, (ii) degradation is prevented by the presence of a cordycepin residue at the 3' end and (iii) RNAs having internally located poly(A) stretches are poor substrates for degradation. The possible involvement of the poly(A) removing enzyme in regulating mRNA translation and stability is discussed.

    A HeLa cell 3'-exonuclease with properties of a mammalian mRNA poly(A) tail-removing enzyme has been characterized. The exonuclease shows high specificity for the poly(A) tail, and it is single strand-specific and requires a 3'-hydroxyl group for its activity. During degradation 5'-AMP is liberated as a product, and a 3'-OH group is left on the last adenosine residue of the remaining poly(A) tail. The activity is inhibited by 5'-AMP and can be competed by poly(A)-containing mRNA or poly(A). Based on these findings we propose a reaction pathway for poly(A) tail removal catalyzed by the HeLa cell poly(A) tail-specific 3' exonuclease.

    Dose Escalation with High Dose Rate Brachytherapy or Protons in Curative Radiotherapy of Prostate Cancer2018Doctoral thesis, comprehensive summary (Other academic)
    Abstract [en]

    The aim of the thesis was to study the outcome and side effects after dose-escalated radiotherapy with high dose rate brachytherapy (HDR-BT) or proton beam therapy (PBT) boost in prostate cancer.

    The first cohorts of men in Sweden treated with either HDR-BT or PBT in combination with conventional photon beam therapy (2 Gray (Gy) fractions to 50 Gy) were analysed. The HDR-BT was given with two 10 Gy fractions, and the PBT with four fractions of 5 Gy. The analyses included 823 men in two HDR-BT cohorts, and 265 men in the PBT cohort. A large proportion of the cohorts, from 38% to 53%, were classified as high risk. After a follow-up between four and eleven years, both combinations showed low risks for relapse. The overall 5-year risk for PSA relapse was 0% for men with low risk. After PBT, the 5-year PSA relapse risk for intermediate and high risk were 5% and 26% respectively. After HDR-BT the 10-year risks for PSA relapse were 0%, 21% and 33% for low, intermediate, and high risk, respectively.

    The risk for early and late toxicity was low. Genitourinary (GU) toxicity was more frequent than gastrointestinal (GI) toxicity. GU toxicity may have a late onset and progress slowly with time after HDR-BT. The 5- and 10-year actuarial incidences of urethral stricture were 6% and 10% respectively after HDR-BT. With applied dose constraints to the urethra the 10-year risk was 5%. The actuarial prevalence of GI toxicity declined slowly with time after HDR-BT as well as after PBT.

    A PSA bounce after HDR-BT was seen in 26% of the patients, more frequent with younger age and lower Gleason score, and followed by a low risk for relapse.

    For dose-escalated radiotherapy with HDR-BT or PBT:

    • long-term tumour control was achieved, not only for low- and intermediate risk, but also for the majority of high risk patients,
    • a PSA bounce after HDR-BT was folled by a good prognosis,
    • levels of late toxicity were low,
    • genitourinary toxicity was more frequent than gastrointestinal toxicity,
    • dose constraints to risk organs must be applied to minimise risks for late toxicity.

    Background and purpose: High dose rate brachytherapy (HDR-BT) in prostate cancer (PC) is receiving increasing interest. The steep dose gradient gives a possibility to escalate the dose to the prostate. If the a/b ratio is low for PC, hypofractionation will be of advantage. A retrospective analysis of outcome in patients (pts) consecutively treated with combined HDR-BT and conformal external beam radiotherapy (ERT) was performed. Material and methods: Data from 214 pts treated consecutively from 1988 to 2000 were analysed. The median age was 64 years (50–77). Median follow up was 4 years (12–165 months). Pre-irradiatory endocrine therapy was given to 150 pts (70%). The pts were divided into low-, intermediate- and high (80/87/47 pts) risk groups according to the occurrence of none, one, or more risk factors defined by T-classification, PSA and histopathology. ERT was given with 2 Gy fractions to 50 Gy. HDR-BT consisted of two 10 Gy fractions. Results: Overall 5-year biochemical no evidence of disease (bNED) was 82%, and for the low-, intermediate-, and high-risk group bNED was 92, 88 and 61%, respectively. PSA-relapse was found in 17, local recurrence in 3 and distant metastases in 13 pts. Five pts died of PC. No recurrences were observed after 5 years. Severe late complications were few. Urethral stricture (13 pts) was the most frequent. No severe rectal complications were seen. Conclusion: Dose escalation with HDR-BT is safe and effective in radiotherapy of localised PC.

    Proton boost of 20 Gy in daily 5 Gy fractions followed by external beam radiotherapy (EBRT) of 50 Gy in daily 2 Gy fractions were given to 278 patients with prostate cancer with T1b to T4N0M0 disease. Fifty-three percent of the patients received neoadjuvant androgen deprivation therapy (N-ADT). The medium followup was 57 months. The 5-year PSA progression-free survival was 100%, 95%, and 74% for low-, intermediate-, and high-risk patients, respectively. The toxicity evaluation was supported by a patient-reported questionnaire before every consultant visit. Cumulative probability and actuarial prevalence of genitourinary (GU) and gastrointestinal (GI) toxicities are presented according to the RTOG classification. N-ADT did not influence curability. Mild pretreatment GU-symptoms were found to be a strong predictive factor for GU-toxicity attributable to treatment. The actuarial prevalence declined over 3 to 5 years for both GU and GI toxicities, indicating slow resolution of epithelial damage to the genitourinary and gastrointestinal tract. Bladder toxicities rather than gastrointestinal toxicities seem to be dose limiting. More than 5-year followup is necessary to reveal any sign of true progressive late side effects of the given treatment. Hypofractionated proton-boost combined with EBRT is associated with excellent curability of localized PC and acceptable frequencies of treatment toxicity.

    Material and methods: The study includes 623 patients, median age of 66 years, treated from 1995 to 2008 and a median follow up of 11 years (range 2–266 months). Androgen deprivation therapy was given to 429 patients (69%). The HDR-BT was given with two 10 Gy fractions and the EBRT with 2 Gy fractions to 50 Gy.

    Results: The 10-year PC-specific survival was 100%, 92%, 91%, and 75% for low-, intermediate-, high- and very high-risk patients respectively, and the 10-year probability of PSA relapse was 0%, 21%, 33%, and 65% respectively. The 10-year actuarial prevalence for ≥grade 2 GU- and GI-toxicities were 28% and 12% respectively and for ≥grade 3, 4% and 1% respectively. Urethral stricture was the most frequent GU complication with a 10-year actuarial incidence of 10%. Treatment without dose constraints for the urethra conferred a higher incidence 18%, compared to 5% after 2003 (p < 0.001). Sixteen patients experienced grade 4 GU toxicity, of which 13 were treated before 2003. No grade 4 rectal toxicity was seen.

    Conclusion: The combination of EBRT and HDR-BT with adequate dose constraints to risk organs provides satisfactory long-term tumour control even in high-risk patients. GI toxicity stabilised but GU toxicity progressed during the 10-year follow up.

    Background

    PSA kinetics after curative radiotherapy for prostate cancer is an important part of the posttreatment evaluation. We analysed PSA bounce occurrence after combined high dose rate brachytherapy (HDR-BT) and external radiotherapy (ERT).

    Material & methods

    We analysed 623 patients treated from 1995 to 2008. The median age was 66 years (47-79). The median initial PSA was 12 ng/ml (0.1-224). Neoadjuvant endocrine therapy was given to 429 patients. ERT was given with 2 Gy fractions to 50 Gy and HDR-BT in two 10 Gy fractions. The median follow-up was 11 years (range 2-266 months). PSA bounce was defined as a temporary rise in PSA >0.2 ng/ml. PSA failure was defined according to the Phoenix definition.

    Results

    PSA bounce occurred in 159 patients (26%), where 56 patients had a bounce amplitude >2 ng/ml and 31 patients had multiple bounces. Median time to bounce peak was 15 (3-103) months with a median bounce value of 1.5 (0.3-12) ng/ml. Younger age and lower Gleason scores were associated with PSA bounce. In a Cox regression analysis with PSA bounce as a time-dependent covariate and adjusted for other prognostic factors, PSA bounce was associated with a lower risk for PSA failure (HR=0.42; 95% confidence interval 0.26-0.70).

    Conclusion

    PSA bounce after HDR-BT combined with ERT is common and associated with a good prognosis. As the relapse risk after an early bounce is very low, the findings should alert clinicians not to initiate salvage treatment too early. Research in prospective identification of PSA bounce is clinically relevant.

    Conclusion: The combination of EBRT and HDR-BT with adequate dose constraints to risk organs provides satisfactory long-term tumour control even in high-risk patients. GI toxicity stabilised but GU toxicity progressed during the 10-year follow up.

    Background

    PSA kinetics after curative radiotherapy for prostate cancer is an important part of the posttreatment evaluation. We analysed PSA bounce occurrence after combined high dose rate brachytherapy (HDR-BT) and external radiotherapy (ERT).

    Material & methods

    We analysed 623 patients treated from 1995 to 2008. The median age was 66 years (47-79). The median initial PSA was 12 ng/ml (0.1-224). Neoadjuvant endocrine therapy was given to 429 patients. ERT was given with 2 Gy fractions to 50 Gy and HDR-BT in two 10 Gy fractions. The median follow-up was 11 years (range 2-266 months). PSA bounce was defined as a temporary rise in PSA >0.2 ng/ml. PSA failure was defined according to the Phoenix definition.

    Results

    PSA bounce occurred in 159 patients (26%), where 56 patients had a bounce amplitude >2 ng/ml and 31 patients had multiple bounces. Median time to bounce peak was 15 (3-103) months with a median bounce value of 1.5 (0.3-12) ng/ml. Younger age and lower Gleason scores were associated with PSA bounce. In a Cox regression analysis with PSA bounce as a time-dependent covariate and adjusted for other prognostic factors, PSA bounce was associated with a lower risk for PSA failure (HR=0.42; 95% confidence interval 0.26-0.70).

    Conclusion

    PSA bounce after HDR-BT combined with ERT is common and associated with a good prognosis. As the relapse risk after an early bounce is very low, the findings should alert clinicians not to initiate salvage treatment too early. Research in prospective identification of PSA bounce is clinically relevant.

    This thesis aims to explore the concept of self-perceived integration by testing assumptions and by introducing new aspect of integration and examining the relations between them. Through interviews with eight persons of different backgrounds, born outside of Europe, who have immigrated to Sweden, it is shown that employment, previously assumed to be of the highest importance for integration, will only lead to further integration if it also provides certain variables. These variables are skills in Swedish, providing a sense of belonging and understanding of the Swedish culture. However, these individuals attest to that unless the individual is motivated enough, and there is mutual respect between the immigrated culture and the Swedish culture, no integration will be possible at all.

    Background: Prostate cancer is the most common form of cancer among men in Sweden.Getting a cancer diagnosis is stressful for both the patient and their partner.Objective: The aim of this study was to investigate the degree of stress responses in the form of avoidance behavior and intrusive thoughts in patients with prostate cancer and their partners. Another purpose was to assure the quality and design of the measuring instrument and its response options.Method: The study had a quantitative cross-sectional design. The data was collected using the questionnaire "Impact of Event Scale" (IES) and by a questionnaire framed according to "The Question and Answer Model" (QAM) to safeguard the quality of the IES. Questionnaires were distributed at surgical and oncology clinics in Uppsala and Falun during the spring of 2013. Inclusion criteria for the study were patients with prostate cancer and their partners (n = 34). 12 respondents participated. The data were analyzed descriptively and with Mann-Whitey test.Results: It was found that patients (n = 6) with prostate cancer have moderate degree of stress responses in the form of avoidance behavior. Partners (n = 6) were found to have strong degree of stress responses in the form of intrusive thoughts. Any significant difference between patients and partners dit not appear. The majority of study participants (n = 11) were confident of their answers in the IES.Conclusion: Partners seem to have a higher level of stress than patients with prostate cancer, which is in line with similar studies' results. Interventions to reach this group and ensure its need is desirable. More studies are lined. The overall results indicate that the measuring instrument IES is of good quality and are measuring the concepts that it indicate to measure.

    Mattias Åström: Jazz, recensioner och konstmusik. En analys av jazzmusikrecensioner och jazzmusikkultur. – Uppsala: Musikvetenskap 2001. 60 p.

    Syftet med uppsatsen är dels att undersöka hur jazzmusikkulturen idag speglas i recensionstexter i dagstidningarna Dagens Nyheter och Svenska Dagbladet och dels att undersöka om sättet att verbalisera jazzmusikrecensioner förändrats mot det konstmusikaliska i takt med att jazzen blivit allt mer accepterad som en form av konstmusik. Recensionerna som analyseras är 15 st. och publicerade 1998 och 1999 i Dagens Nyheter och Svenska Dagbladet. Den tidigare forskning på området som refereras är Sirpa Koiranens avhandling, Språk, musik och kultur, utgiven 1992. I uppsatsen används delar av Koiranens analysmetod – det som hon kallar för ramelement. De står för de allmänna förväntningarna som finns på en konsert och fungerar som ett slags minsta gemensamma nämnare för olika konsertsituationer och olika musikkulturer. Ramelementen är verktyg för att bena upp de olika recensionstexterna och kunna jämföra dem med varandra.

    I det första kapitlet förklaras sambanden mellan språk och kultur. I det andra redogörs för jazzmusikens förhållande till kulturen i stort och begrepp som diskuteras i samband med jazzmusikkulturen är sub- och motkulturbegreppen, status och konstmusik.

    I uppsatsen visas att de skillnader mellan de två musikkulturernas språkbruk som Koiranen kom fram till i sin undersökning i stort finns kvar. I uppsatsen visas dock att språket i jazzmusikrecensionerna på olika sätt har förändrats, bl.a. är språket neutralare. Dessa förändringar kan möjligen ha ett samband med jazzens ökade status och erkännande som en form av konstmusik. I uppsatsen diskuteras och ifrågasätts också om jazzrecensioner bär spår av motkultur i förhållande till konstmusikrecensioner, vilket Koiranen hävdade i sin avhandling.

    Länge har optionsprogram använts som en ersättningsform inom amerikanska bolag för att få den verkställande direktören att arbeta i enlighet med bolagets intresse. Genom optionsprogrammen knyts bolagets lönsamhet till den verkställande direktörens prestation. Den verkställande direktören får därmed ett incitament att ta risker och maximera värdet på bolagets aktier. Flera vetenskapliga studier som har genomförts på amerikanska bolag visar även att risken ökar när den verkställande direktören ges optioner i bolagets aktier. Det finns däremot inte några motsvarande studier när det gäller svenska förhållanden. Till skillnad från de amerikanska bolagen karaktäriseras svenska bolag av en stark ägarkoncentration med en hög grad av övervakning och insyn vilket skulle kunna påverka den verkställande direktörens risktagande. Studiens syfte är därför att undersöka om det finns ett samband mellan optionsprogram och förändring i total risk för svenska bolag noterade på Nasdaq OMXS. Resultatet antyder att den totala risken minskar när den verkställande direktören ges optioner vilket därmed skulle kunna tyda på att de svenska bolagens speciella karaktäristika har en betydelse eller att användningen av optionsprogrammen ännu inte är optimalt utformade i Sverige.

    Abstract [en]

    While development cooperation largely consists of policy advice, the suggestions and inputs on development that policymakers in the Global South receive from donors and other international actors, little research exists on what makes policy advice useful to these policymakers. The aim of this study was to find out why policymakers in the Global South find some advice useful and some not. This was done through a field study in Kenya, using interviews with 23 policymakers and advisors. The analytical framework that was applied to the material reconciled the SCL model of analysing policy advice in terms of its content, with a critical postcolonial perspective which regarded the delivery of advice. The study showed that  the usefulness of policy advice for Kenyan policymakers is as dependent on the delivery of the advice as it is on its content, unlike what has been implied previously by Western research on think tanks. The study also concluded that not only is it possible to merge the two theoretical perspectives of policy advice and postcolonial theory, despite the difference in abstraction levels, applying a postcolonial perspective can be crucial in order to examine the full scope of what makes policy advice useful to policymakers in the Global South.

    I uppsatsen undersöks hur genetiska grödor uppfattas i sammanhanget av dem som en lösning på hungerfrågan. Analysens material består av artiklar publicerade i svensk dagspress från januari 2008 till och med november 2011. De artiklar som ingått i studien har alla berört hungerfrågan och hur gentekniken kan påverka denna. I uppsatsen har analysmetoden kritisk diskursanalys använts, denna har kompletterats med biopolitiskt teori. Frågeställningar i uppsatsen var;Vilka diskurser finns i tal om GMO som lösning på hungerfrågan? Hur konstrueras dessa diskurser? Hur kan dessa diskursers effekter på samhället förstås ur ett biopolitiskt perspektiv?Analysen visar att det råder en motståndsdiskurs mot GMO som lösning på hungerfrågan och en förespråkande diskurs. Den förespråkande diskursen har konstruerats genom en mängd möjlighetsdiskurser om GMO. Motståndsdiskursen har konstruerats genom riskdiskurser, men även möjlighetsdiskurser i form av ekologiska diskurser och diskurser om välfärd. Ur ett biopolitiskt perspektiv framstår inte den genmodifierade grödans vara eller icke-varande som fullt så betydelsefull för hungerfrågan som låts påskina i texterna, istället framstår biomakten som avgörande för hungerfrågan. Då Foucault menar att orättvisor, ojämlikhet och segregation är en del av biomakten (2002:142) därigenom kan frågan ställas om existensen av GMO eller dess motstånd löser hungerfrågan.

    I denna uppstats görs en idéanalys av det innehåll Moderaterna respektive Socialdemokraterna fyllt konceptet “arbetslinjen” med sedan millennieskiftet och vad detta implicerar för syn på de arbetslösa. Ett antal arbetsmarknadspolitiska propositioner och protokoll från tillhörande riksdagsdebatter studeras och analyseras med hjälp av “idealtyper” av olika varianter av “arbetslinjen”. Resultaten visar att Socialdemokraterna har fört en arbetslinje med fokus på samhällets skyldighet att ge de arbetslösa stöd att få ett arbete. Ett fokus har också funnits på att arbetena ska vara välbetalda och passa de arbetslösa. En positiv bild av de arbetslösa där de ses som offer för arbetslöshet impliceras. Moderaterna har fört en arbetslinje som överlag också varit inriktad på att ge de arbetslösa stöd, men som haft mer fokus på eget ansvar och att motverka missbruk av ersättningar. Vid ett tillfälle synes de varit mer fokuserade på att kräva motprestation från de arbetslösa än att på att ge dem stöd. En dubbel bild av de arbetslösa, där de å ena sidan ses som offer för arbetslöshet som bör ges stöd och å andra sidan ses som “skyldiga” till arbetslöshet vilka bör pressas, impliceras.

    The authors review the efforts made from a modeling and simulation perspective in order to assist both the fundamental understanding as well as the development of higher performance sodium-ion battery (SIB) electrolytes. Depending on the type of the electrolyte studied, liquid, ionic liquid, polymer, glass, solid-state, etc., the simulation methods applied and the research questions in focus differ, but all contribute to more rational progress. Furthermore, the authors create cases of meta-analysis using literature data. A historical perspective is applied and the focus clearly is on more recent work and novel electrolyte materials. Finally, the authors outline a few prospective areas for where SIB electrolyte simulations can/should be extended for maximum impact in the field.

    Molecular and Cellular Complexity of Glioma: Highlights on the Double-Edged-Sword of Infiltration Versus Proliferation and the Involvement of T Cells2012Doctoral thesis, comprehensive summary (Other academic)
    Abstract [en]

    Glioblastoma multiforme (GBM), the most common and malignant brain tumor, is characterized by high molecular and cellular heterogeneity within and among tumors. Parameters such as invasive growth, infiltration of immune cells and endothelial proliferation contribute in a systemic manner to maintain the malignancy.

    Studies in this thesis show that the expression of Sox2 is correlated with Sox21 in human gliomas. We demonstrate that an upregulation of Sox21 induces loss of proliferation, apoptosis and differentiation in glioma cells in vitro and in vivo and seems to correlate with decreased Sox2 expression. Induced expression of Sox21 in vivo significantly reduces the tumor size and increase the survival extensively, suggesting that Sox21 can act as a tumor suppressor Our studies indicate that the balance of Sox21-Sox2 in glioma cells is decisive of either a proliferative or a non-proliferative state.

    Several TGFß family members have an important role in glioma development. TGFß promotes proliferation and tumorigenicity whereas BMPs mostly inhibit proliferation. We demonstrate that BMP7 can induce the transcription factor Snail in glioma cells and that this reduces the tumorigenicity with a concomitant increase in invasiveness. Thus, we have identified a mechanism to the double-edged sword of proliferation versus invasiveness in GBM, the latter contributing to relapse in patients.

    Experimental gliomas were induced with the Sleeping Beauty (SB) model in mice with different immunological status of their T cells. The tumors that developed were either GBMs or highly diffuse in their growth, reminiscent of gliomatosis cerebri (GC). GC is a highly uncommon form of glioma characterized by extensive infiltrative growth in large parts of the brain. It is an orphan disease and today there is practically a total lack of relevant experimental models. The SB system would constitute a novel experimental model to study the mechanisms behind the development of diffusely growing tumors like GC. The presence or absence of T cells did not affect tumor development.

    The work in this thesis demonstrates that the proliferative and the invasive capacities of glioma cells can be dissociated and that the SB model constitutes an excellent model to study the highly proliferative cells in GBMs versus the highly invasive cells in diffuse tumors like .GC.

    Numerous studies support a role for Sox2 to keep stem cells and progenitor cells in an immature and proliferative state. Coexpression of Sox2 and GFAP has been found in regions of the adult brain where neural stem cells are present and in human glioma cells. In our study, we have investigated the roles of Sox2 and its counteracting partner Sox21 in human glioma cells. We show for the first time that Sox21 is expressed in both primary glioblastoma and in human glioma cell lines. We found that coexpression of Sox2, GFAP and Sox21 was mutually exclusive with expression of fibronectin. Our result suggests that glioma consists of at least two different cell populations: Sox2+/GFAP+/Sox21+/FN- and Sox2-/GFAP-/Sox21-/FN1+. Reduction of Sox2 expression by using siRNA against Sox2 or by overexpressing Sox21 using a tetracyclineregulated expression system (Tet-on) caused decreased GFAP expression and a reduction in cell number due to induction of apoptosis. We suggest that Sox21 can negatively regulate Sox2 in glioma. Our findings imply that Sox2 and Sox21 may be interesting targets for the development of novel glioma therapy.
