    Residential Mobility and Tenure Choice in the Swedish Housing Market2001Doctoral thesis, comprehensive summary (Other academic)
    Abstract [en]

    This thesis consists of three self-contained essays concerned with the microeconomics of housing.

    In Essay 1 I examine the determinants of residential mobility among Swedish households within the framework of duration models. The results give support to the widely acknowledged life cycle - housing cycle connection. Demographic factors are important; age, household size, and the stability of household size all influence residential mobility to a great extent. The notion of a housing career is very plausible. The analysis also indicates that economic factors are important. Changes in the economic environment may disrupt the planned housing career. A higher household income tends to shorten stays in a particular dwelling. An increased cost of owning a home reduces the probability that a renter household should move. This might be interpreted as evidence that the planned housing career is disturbed, i.e. the move from renting to owning is postponed for many households.

    Essay 2 expands on the findings in essay 1 in that the link between the stage in the life cycle and the choice of housing tenure is looked at more closely. The main questions that I address in the second paper are how changing circumstances in the domestic sphere and on the labor market affect transitions between different housing tenures, and thereby also the resulting housing tenure distribution in the economy. On the individual level these factors are found to influence tenure transitions greatly. However, I also find that the resulting long-term distribution of households over housing tenures is little affected by increases or decreases in the arrival rate of these events.

    In Essay 3 I explore how the pattern of owner occupation has evolved during the period 1970-97, and examine how a few "typical" households have been able to further their housing situation during this period. Results from the study show that the pattern of owner occupancy differs markedly between age cohorts; households today are much more likely to go through a housing career than they were 30 years ago. Moreover, gender effects vanish over time and household composition is not as important for home ownership in the end of the period as in the beginning. Results also show that the negative impact of being foreign born on the probability of owner occupancy has increased since 1970, and that the influences of household income and relative cost of owned housing have become more pronounced over time.

    Abstract [en]

    These studies were undertaken to investigate risks of maternal and perinatal complications in pregnant women with chronic hypertensive disease, and to investigate future risk of preeclampsia in women born small for gestational age (SGA). Population based cohort studies using the Swedish Medical Birth Register from different years were performed.

    The maternal complications mild and severe preeclampsia, gestational diabetes and abruptio placenta were studied in a population of 681 515 women, with a prevalence of 0,5% for chronic hypertension. Risk estimates were adjusted for differences in maternal characteristics as age, parity, BMI, ethnicity and smoking habits. Chronic hypertensive women wore found to have significantly increased risks of all complications.

    The perinatal complication SGA was studied in a population of 560 188, with a prevalence of 0,5% for chronic hypertension. Risk estimates were adjusted for differences in maternal characteristics and for the secondary complications mild and severe preeclampsia. Chronic hypertensive women were found to suffer a significantly increased risk of giving birth to an offspring that is SGA.

    The perinatal complication fetal/infant mortality was studied in a population of 1 222 952 with a prevalence of 0,6% for chronic hypertension. Risk estimates were adjusted for differences in maternal characteristics and for the complications mild and severe preeclampsia, gestational diabetes, abruptio placenta and offspring being SGA In the analysis an effect modification by gender was included. Chronic hypertensive women were found to have a significantly increased risk for stillbirth and neonatal death in male, but not in female, offspring. Thus a clear gender difference in mortality was revealed. The risk of mortality of offspring was mediated by severe preeclampsia, abruptio placenta and offspring being SGA. Mild preeclampsia and gestational diabetes did not affect the risk. No increased risk of post neonatal mortality was found.

    A generation study was performed in 118 634 girls of which 5.8% were born SGA. Their future risk for mild and severe preeclampsia in first pregnancy was analysed. Risk estimates were adjusted for age, smoking, BMI and for preeclampsia in the mothers while pregnant with the study population. Women who were born SGA were shown to have a significantly increased risk for severe preeclampsia, but not for mild preeclampsia.

    Objective: The first aim of this study was to investigate the risk of pre-eclampsia, both mild and severe, in women born small for gestational age (SGA). The second aim was to investigate whether the risk is modified by pre-eclampsia in the previous generation. Design: Population-based cohort study. Setting: Sweden. Population: A population of 118 634 women registered both as newborns and as mothers in the Swedish Medical Birth Register of 1973-2003. Of these, 6883 had been born SGA. Only primiparas and singletons were included. Methods: The pregnancies that the women were born out of were analysed with regard to presence of pre-eclampsia, while their own pregnancies were analysed regarding age at delivery, smoking, body mass index and incidence of mild or severe pre-eclampsia. Multiple logistic regression analysis was used. In a first step, we adjusted for maternal characteristics, and in a second step, for pre-eclampsia in the previous generation. Main outcome measures: Odds ratio for mild and severe pre-eclampsia. Results: In women born SGA, the adjusted odds ratio (first step) for mild pre-eclampsia was 1.19 (95% CI 1.03-1.38), while for severe pre-eclampsia it was 1.69 (95% CI 1.40-2.02) compared with those not born SGA. After the second-step adjustment, the odds ratio for mild pre-eclampsia was 1.16 (95% CI 1.00-1.35) and for severe pre-eclampsia was 1.62 (95% CI 1.35-1.95). No statistically significant effect modification from pre-eclampsia in the previous generation was shown. Conclusions: Women born SGA suffer a markedly increased risk of severe pre-eclampsia. Exposure to pre-eclampsia during a woman's own fetal development significantly increases her risk of pre-eclampsia but does not modify the SGA effect.

    Abstract [en]

    Objective: The first aim of this study was to investigate the risk of pre-eclampsia, both mild and severe, in women born small for gestational age (SGA). The second aim was to investigate whether the risk is modified by pre-eclampsia in the previous generation. Design: Population-based cohort study. Setting: Sweden. Population: A population of 118 634 women registered both as newborns and as mothers in the Swedish Medical Birth Register of 1973-2003. Of these, 6883 had been born SGA. Only primiparas and singletons were included. Methods: The pregnancies that the women were born out of were analysed with regard to presence of pre-eclampsia, while their own pregnancies were analysed regarding age at delivery, smoking, body mass index and incidence of mild or severe pre-eclampsia. Multiple logistic regression analysis was used. In a first step, we adjusted for maternal characteristics, and in a second step, for pre-eclampsia in the previous generation. Main outcome measures: Odds ratio for mild and severe pre-eclampsia. Results: In women born SGA, the adjusted odds ratio (first step) for mild pre-eclampsia was 1.19 (95% CI 1.03-1.38), while for severe pre-eclampsia it was 1.69 (95% CI 1.40-2.02) compared with those not born SGA. After the second-step adjustment, the odds ratio for mild pre-eclampsia was 1.16 (95% CI 1.00-1.35) and for severe pre-eclampsia was 1.62 (95% CI 1.35-1.95). No statistically significant effect modification from pre-eclampsia in the previous generation was shown. Conclusions: Women born SGA suffer a markedly increased risk of severe pre-eclampsia. Exposure to pre-eclampsia during a woman's own fetal development significantly increases her risk of pre-eclampsia but does not modify the SGA effect.

    Background. Chronic hypertension during pregnancy is associated with an increased risk for birth of small for gestational age offspring. The aim of this study was to determine whether the risk remains after consideration of maternal characteristics and superimposed pre-eclampsia. Method. A population-based cohort study based on the Swedish Medical Birth Register 1992-98 and comprising 560,188 women aged 15-44 years with singleton pregnancies including 2,754 women with chronic hypertension. The register contains data of maternal characteristics besides maternal and fetal complications. Multiple logistic regression analysis was used. Small for gestational age was defined as birth weight less than -2SD adjusted for gestational age and sex. Results. Chronic hypertensive women have an independent risk for small for gestational age (OR 3.1; 95% CI 2.7-3.7) when controlling for confounding of maternal characteristics such as age, parity, BMI, smoking, and ethnical origin. After introducing superimposed pre-eclampsia in the model the risk remains but decreases (OR 2.4; 95% CI 2.1-2.9). Conclusion. Chronic hypertension is an independent risk factor for birth of small for gestational age offspring. Pre-eclampsia is a strong mediating factor.

    OBJECTIVE: The purpose of this study was to determine whether there is a difference, by gender, in perinatal mortality in chronically hypertensive women compared with normotensive women. DESIGN: Population-based prospective cohort study. SETTING: Sweden. POPULATION: A total of 866,188 women with singleton pregnancies registered in the Swedish Medical Birth Registry 1992-2004, of which 4749 were diagnosed with chronic hypertension. METHODS: Multivariate logistic regression analysis was performed. In a first step, we adjusted for maternal characteristics and in a second step for mild and severe pre-eclampsia, gestational diabetes, placental abruption and small for gestational age. An effect modification by gender was included in the model. MAIN OUTCOME MEASURES: Odds ratios (OR) for intrauterine death, neonatal death and post-neonatal death with respect to gender of offspring. RESULTS: The unadjusted OR of intrauterine death was 4.12 (95% CI: 2.84-5.96) and 1.29 (95% CI: 0.67-2.48) for male and female offspring, respectively, and of neonatal death, it was 3.45 (95% CI: 2.13-5.59) and 2.17 (95% CI: 1.08-4.35) for male and female offspring, respectively. After multivariate analysis, the OR of intrauterine death was 3.07 (95% CI: 2.12-4.46) and neonatal death was 2.99 (95% CI: 1.84-4.85) for male offspring. For female offspring, the OR of intrauterine death was 0.98 (95% CI: 0.51-1.89) and neonatal death was 1.88 (95% CI: 0.93-3.79). CONCLUSION: Mothers with chronic hypertension have an increased risk of perinatal mortality of their male offspring.

    Introduction: Overweight and obesity is a globally growing public health problem leading to enormous economic costs for the society. Obesity surgery, including Adjustable Gastric Banding (AGB), has shown long-term positive effects on weight loss, while non-surgical obesity treatment has shown less evident effects. This has led to a dramatic increase in obesity surgery. Still, some surgery patients have difficulties in maintaining weight loss over time. The majority of experts argue that long-term treatment is essential for maintaining behavioral changes. According to experts, Daycare treatment is worth considering for patients suffering from obesity. It has been used broadly within the diabetes care though not to the same extent in obesity treatment. Aim: To structure and investigate the population and data from an already completed but unpublished three year old intervention study, with three follow ups, of weight loss and eating behavior in patients with obesity after treatment with Adjustable Gastric Banding, Daycare or a combination of the both treatments. Method: Data in 30 binders, from participants (baseline n=212) in an already completed but unpublished intervention study, was read and structured into an index in Microsoft Words. A database in Microsoft Excel was then structured were patients with specified measures for weight loss (BMI) and eating behavior (TFEQ), for one, two or three of the total three follow ups, was counted together. Results: After the structuration and investigation of the material it became evident that due to drop-out or missing data only (n=10) out of (n=212) had fulfilled the study by showing specified measures of BMI and TFEQ at every of the total three follow ups. To, in the present Master thesis, show results from a greater sample, also participants with measures (BMI, TFEQ) from baseline and the third follow up, but not necessary from the first or second follow up (n=43), was included in the presentation. BMI and TFEQ were presented in mean and median for each of the treatment groups. Conclusion: The result shows that a majority of the participants not completed the program. Hence could no effect of the treatments on changes of weight and eating behavior be rated.
