    In-bed cycle exercise for mechanically ventilated burn patients. Five case reports.

    Objectives: The aim of the study was to assess the impact of motorized passive or active in-bed cycle exercise, for mechanically ventilated burn patients, on heart rate (HR), mean arterial pressure (MAP), respiratory rate (RR), and blood gas values and to evaluate if the intervention is possible to execute for 20 minutes, as recommended for other ICU patient groups.

    Methods: A Single-Subject Experimental Design study was conducted with five consecutively included critically ill adult burn patients on mechanical ventilation (33-88 years of age, four were men, TBSA% burned 12-58%). Data were collected during the participants’ first session of passive in-bed cycle exercise. The intervention was conducted within 1-26 days (median 3) after burn injury.

    Results: A comparison between mean baseline measurements and mean values during intervention show an increase in HR for all participants (2.7 (range: 0.3 to 5.5) bpm). An increase between 4 and 12 mmHg in MAP was seen in three participants and a decrease of around 5.5 mmHg in two participants. Three participants increased their mean RR (range: 1.7 to 5.5 breaths per minute), one decreased (0.8), and one had controlled mechanical ventilation and thus no change. Arterial blood gases showed only minor changes.

    Conclusion: All participants managed to maintain the passive in-bed cycle exercise for 20 minutes, without deviating from the individually preset thresholds for aborting the intervention. Early in-bed cycling exercise for mechanically ventilated burn patients at the Burn intensive care unit is considered to be a clinically safe procedure since effects on the physical variables were negligible. However, further investigations are needed.

    Abstract [en]

    Building on the core syllabus for postgraduate training in respiratory physiotherapy, published in 2014, the European Respiratory Society (ERS) respiratory physiotherapy task force has developed a harmonised and structured postgraduate curriculum for respiratory physiotherapy training. The curriculum outlines the knowledge, skills and attitudes which must be mastered by a respiratory physiotherapist working with adult or paediatric patients, together with guidance for minimal clinical exposures, and forms of learning and assessment. This article presents the rationale, methodology and content of the ERS respiratory physiotherapy curriculum. The full curriculum can be found in the supplementary material.
