Exosomes are a promising class of biomarkers for diagnosing and monitoring cancer and other diseases, but methods have been lacking to establish diagnostic assays for exosomes. These microvesicular structures are typically secreted by epithelial cells in an exocrine fashion, but in invasive disease they may not reach the lumen, resulting in elevated blood levels. The purpose of this project is to further develop a proximity ligation assay uniquely suited to detect complexes of multiple proteins as required for exosome detection. We will extend our earlier observation that a class of exosomes called prostasomes occurs at elevated levels in prostate cancer patients, and may provide improved discrimination among patients and controls compared to PSA. We will also broaden the investigation to include a number of tissue-specific exosomes that may furnish new diagnostic markers in a wide range of common malignant and nonmalignant diseases, improving opportunities for early diagnosis.
