Ruptured abdominal aortic aneurysm (AAA) is a common preventable cause of death in developed countries, 700-1.000 deaths are caused by this disease annually in Sweden.Most patients who suffer a rupture of an AAA have no symptoms prior to rupture, and the mortality is 80-90%. The disease can easily be detected by ultrasound. Four different randomized controls trials (RCT) were performed comparing screening elderly men for AAA with non-screening. Aneurysm related mortality was reduced by 50-70%, and all-cause mortality was also reduced significantly. It has also been shown that screening for AAA is cost-effective.After a pilot study we started the first population-based AAA screening program in Sweden in the County of Uppsala in 2006, by inviting all 65-year old men. The program spread rapidly, and we estimate that 90% of all Swedish 65 year old men will be invited during 2011. Our preliminary results 2006-2009 showed a prevalence of AAA of 1.5% (defined as a diameter above 29 mm), and a compliance of 83%. Another 1.5% had an aortic diameter 25-29mm.A lower prevalence of AAA than expected, together with a longer life-expectancy compared to the population in the RCTs, makes it important to study the disease in a contemporary Swedish context. We intend to rescreen this cohort at the age of 70 and 75. How many of the new AAAs develop in the group of men who have an aortic diameter of 25-29 mm? Other risk-groups? How does rescreening affect cost-effectiveness?
