Ruptured abdominal aortic aneurysm (AAA) is a common preventable cause of death in developed countries, causing an estimated 700-1.000 deaths annually in Sweden. Most patients with a rupture of an AAA have no symptoms prior to rupture, the mortality is 90%. The disease can easily be detected by ultrasound. Four different randomized controls trials were performed comparing screening elderly men for AAA with non-screening. Aneurysm related mortality was reduced by 50-70%, all-cause mortality was reduced significantly. It has been shown that screening for AAA is highly cost-effective.After a pilot study the first population-based AAA screening program in Sweden started in Uppsala in 2006, inviting all 65-year old men. The program spread and in 2011 >90% of all Swedish 65 year old men were invited. Results from screening 22.187 men in five counties in Middle Sweden showed a prevalence of AAA of 1.7% (defined as a diameter >=30 mm), another 0.5% had a known AAA prior to screening. The compliance was 85%. Another 1.8% had an aortic diameter 25-29mm. A lower prevalence of AAA than expected, a longer life-expectancy compared to the population in the RCTs and improved treatment, make it important to study the disease in a contemporary context.We intend to rescreen this cohort at the age of 70 and 75 years. How many of the new AAAs develop in the group of men with an aortic diameter of 25-29 mm at the age of 65? Other risk-groups? How does rescreening affect cost-effectiveness?
