Abstract [en]
Improved detection and diagnostic work-up have led to a large increase in the number of men diagnosed with prostate cancer (Pca) and a strong shift in clinical presentation. To reap the benefit of early detection of ¬†lethal Pca while avoiding overtreatment of indolent cancer, active surveillance, a novel treatment strategy has¬† been developed. In this strategy a man is followed closely and only if/when signs of progression occurs will curative treatment be recommended. Active surveillance has been shown to be safe up to 15 years after diagnosis. However, many men who currently select active surveillance have a life expectancy up to 30 years and outcomes of this strategy have not been demonstrated for this extended time span. We propose to investigate outcomes of active surveillance up to 30 years by use of state transition modelling. Next,¬†the underlying causes of the ¬†decrease in Pca mortality will be investigated. We will use ‚`big data’ in a ¬†nationwide population-based database with comprehensive data on cancer characteristics and cancer as well as¬†data on comorbidity, drug use, familiar aggregation, and socioeconomic status. This project will provide proof-of principle that advanced statistical models applied to big data can provide information on effects of changes in health care policy. Given ¬†increases in screening and detection of early stages of other cancers and many chronic diseases, our methods will therefore also be useful in other areas of health care.
