Background: Patient-reported outcome measures (PROMs) are questionnaires used to evaluate a patient’s health status based on the patient’s own assessment. PACT-M is a PROM developed in the United Kingdom (UK), used to evaluate the quality and safety of care transitions from hospital to own home.

Aim: To translate and culturally adapt the English PACT-M into a Swedish version. The translation and cultural adaptation is to be performed using a validated and established process.

Methods: A process consisting of 10 steps was conducted for translation and cross-cultural adaptation of the PACT-M. The process was adapted from the ISPOR Principles of Good Practice for the Translation and Cultural Adaptation Process for Patient-Reported Outcome (PRO) Measures, with contribution from the Guidelines for the Process of Cross-Cultural Adaptation of Self-Report Measures. The material subjected for translation was sourced from Oikonomou et al. (2020). A Swedish pre-final version was created, using 5 individuals acting as translators in different steps of the process. Focus group interviews were held, with a total of 11 participants to debrief the Swedish pre-final version using a qualitative method for testing the interpretation and comprehensibility of the questionnaire.

Results: Changes to the pre-final version were based on the outcome of the focus group interviews and later finalised into a Swedish version of the PACT-M. The result from the focus group interviews showed difficulties with the translation and adaptation of certain words such as the translation of the English word “family member”, which in Swedish can be either “anhörig” or “närståede”.

Conclusions: Using a suitable method for the translation and cross-cultural adaptation, the Swedish version of the PACT-M was produced. Additionally, a comprehensive translation report was created as a result of the translation and cross-cultural adaptation of the PACT-M.
